**About Position**:
Sahara Addictions Program provides assistance and support to clients and their families dealing with addiction challenges. The program focuses on supportive counseling, and outreach to increase access to additional community-based services using various therapeutic approaches, guided self-change, relapse prevention, link to primary care if needed, follow-up, and aftercare. The goal is to have access to holistic supports that address the underlying factors leading to addiction. The Addictions program accepts referrals from hospitals, community organizations, health practitioners, families, friends, or self-referrals.

The Addictions Case Manager will provide initial and ongoing assessment, treatment planning, brief intervention, and supportive counseling to assist the individual in developing skills to manage substance abuse and related problems. Services will be provided in individual, family, and group formats. Activities will include addiction treatment, family intervention, relapse prevention, follow-up, and aftercare.

**Qualifications and skills required**:

- University Degree or College diploma in social services or health services field is required
- Preferred Registered Health professionals and adhere to standards of practice as per designated college
- Preferred 2 years of experience in the related field, or a combined equivalent of education and experience
- Demonstrate knowledge about addiction treatment services and available community resources
- Proven knowledge and skills in assessment, care plan development, case management and supportive counselling
- Familiarity with the Drug Addiction Treatment Information System (DATIS), GAIN Q3-MI is an asset
- Consistent commitment to professional development
- Strong interpersonal skills, be able to work independently and within a team
- Strong verbal and written communication skills including presentation skills and the ability to communicate information to a variety of stakeholders effectively
- Excellent computer skills with advanced proficiency in Microsoft Office
- Familiarity with Cloud technology and SharePoint will be an asset

**Requirements**:

- Knowledge of Punjabi, Hindi and Urdu languages is required
- Must possess a valid Ontario Driver's License, have regular access to a reliable vehicle and provide proof of adequate vehicle insurance
- A satisfactory current Vulnerable Sector Screening (Criminal Record Check) report
- Current CPR and First-Aid certification
- Vaccines (COVID-19 and others) are encouraged

Key Responsibilities:

- Completion of screening, orientation, intake and assessment with clients and family members.
- Support clients in reaching their goals through interventions such as advocacy, referral to appropriate community resources/programs, supportive counselling, family support and psychoeducation.
- Case management and group facilitation for clients with Addiction concerns.
- Conduct risk and psychosocial assessments using validated tools
- Focus on client engagement and retention.
- Supporting clients and families with problem-solving, care-plan/ treatment planning, crisis intervention and mediation.
- Provide on-going supportive counselling, and appropriate referrals.
- Consult, advocate and liaison with community resources and agencies to assist clients in achieving their goals.
- Maintain current knowledge and positive relationships with various community partners and resources
- Support clients to successfully work towards their care plan and ensure all appropriate supports are in place to transition fully into the community
- Build positive relationships with appropriate informal support as identified by clients
- Implementation of the Integrated Holistic Service Delivery Model
- Participate in program strengthening, restructuring and ensuring that programs are in compliance with CARF Accreditation requirements.

- Maintain appropriate record-keeping/documentation and client files, statistical, and outcome measurement tools as required by PCHS policies and procedures.
