**MDS Coordinator Job Description**
Reports directly to DON and works closely with the Clinical Manager (CM) and is responsible for maintaining accurate data entry of resident information, including accurate coding of the MDS and submission of required documentation to CIHI. Works closely with the Home's interdisciplinary team, residents and their family members to obtain and ensure accuracy of resident information.
**RESPONSIBILITIES**:
- Monitor MDS and care plan documentation for every resident. Ensure that the resident care documentation is reviewed through quarterly audits ensuring care plans accurately reflect resident care requirements;
- Ensure documentation is present in the Clinical Records to support MDS Coding;
- Support the tracking system of MDS schedule (time frames and due dates);
- Ensure frequent and accurate data entry of resident information into appropriate computerized MDS program;
- Coordinate/save back-up information, including updating information manually in case of computer breakdown and/or power outage;
- Complete accurate coding of the MDS with information obtained via medical records review, observations, interviews with facility staff, residents and family members. Ensure timely opening/closing assessments, coding, raps, care planning and timely submissions to CIHI;
- Attend interdisciplinary team meetings and other related functions in order to gather information, communicate changes, necessary to maintain up-to-date information, as directed by the DON/Designate;
- Evaluate effectiveness of clinical records to ensure adherence with documentation policies and procedures, College of Nurses of Ontario protocol and the legislated requirements for long term care facilities in Ontario and other regulations, as required. Assist DON/CM to develop corrective action plan and follow-up compliance measures;
- Attend educational programs in order to upkeep and maintain knowledge base related to data entry and computer technology as directed by the MLTC and DON/Staff Educator.
- Coordinate completion and electronic quarterly submission of required reports/documents to CIHI and other entities per company policy;
- Actively participate in Quality Improvement activities, as may be directed by the DON;
- Oversee the RAI-MDS process according to applicable policies, laws and agreements, Standards of Practice, and interRAI requirements in collaboration with Staff Educator;
- Represent the Home at appropriate community and professional meetings; attend workshops, in-service sessions and other scheduled meetings as authorized by the DON;
- Attend the Data Submission self-study training by CIHI
- In cases of staff shortages, replace staff on the unit as directed by the DON/designate, including emergency situations.
- Work closely with the physiotherapist ensuring broad range of interventions which maximize resident potential for independence, improved mobility, transferability and pain relief.
- Oversee the implementation and maintenance of individual and group programs as developed by the Physiotherapist including ambulation exercise and pain relief.

**Experience/ Education**:
- Must be a Registered Nurse (RN) or Registered Practical Nurse (RPN) with current Registration with the College of Nurses of Ontario, and have completed RN/RPN program requirements. Credentialing for MDS/RAI is required as per MLTC guidelines to include:
- required training from CIHI to implement the RAI-MDS Tool (User Manual RAI-MDS 2.0 may be obtained from CHIHI by contacting the education desk. The required RAI-MDS training is provided by CIHI at no cost and is outlined in the CIHI Learning & Development interRAI in Continuing Care info sheet.
