Jean Tweed Centre is a leading, community-based organization that provides a safe and supportive environment for women with substance use, mental health, and gambling concerns. Recognizing the diversity of women’s experience in today’s society, our programs seek to address a range of issues in a woman’s life that may contribute to substance use and gambling concerns. Understanding each woman’s experience within the broader physical, emotional, social, cultural and gender framework, we provide a client-centered approach to health.
The Case Manager will provide encouragement, emotional support, help and advice on practical matters to clients. Coordinates support and outreach activities that may include: coordination of substance use support and related services, motivational counselling, brief assessment and referral, conducting education and information services, supporting daily activities of clients.
**Our Justice Addiction and Mental Health Program vision**: The Individual in the Justice Addiction and Mental Health position will provide outreach, court accompaniment and support, counselling, referral and case management services to women with concurrent mental health and substance use concerns who also have current involvement in the criminal justice system. Case management duties include liaising with other mental health, substance use services as well as probation/parole, physicians, and community workers. Working across sectors (Justice, Addiction & Mental Health), this community support worker will work to reduce barriers for this population through the provision of both direct and indirect service
Responsibilities Service
- Establish a comprehensive, wholistic, individualized service plan with each client on your caseload.
- Lead a collaborative wrap-around process to ensure comprehensive service coordination that reflects individual needs.
- Provide short term case management support including counselling, outreach, on-going assessment, goal planning, referral, development of support systems, system navigation, etc.
- Provide linkage to long term supports including mental health and justice permanent housing.
- Liaison with Probation and Parole, Drug treatment court and other Mental health and justice providers
- Complete required case notes, evaluations, and data entry reports in an accurate and timely manner.
- Conduct job responsibilities with a commitment to harm reduction, trauma
- informed, women-centred and anti-oppressive practices.
**Administration**
- Maintain documentation of relevant client interactions and interventions, and contacts with collateral supports.
- Monitor the progress of clients, prepare reports and letters as necessary.
- Participate in staff meetings, team development and supervision meetings.
- Collaborate with supervisor to discuss job performance and establish professional goals.
- Engage in ongoing staff development and personal learning as agreed to in performance reviews.
**Organizational**
- Conduct job responsibilities with a commitment to harm reduction, trauma
- informed, women-centred and anti-oppressive practices.
- Abide by the Occupational Health and Safety Act, and work in a manner that is safe, reporting incidents immediately to direct supervisor.
- Adhere to the policies and procedures of The Jean Tweed Centre.
- Perform other duties as assigned in order to meet the overall goals and objectives of The Jean Tweed Centre.
- Maintain a client service orientation including learning about and addressing client needs and concerns.
- Work collaboratively including respect others, sharing information, consulting and celebrating shared successes.

**Qualifications**
- Bachelors Degree in Social Work, or a related discipline.
- A combination of 3 years of work experience in the field of Mental Health, Substance Use, Criminal Justice and/or related field.
- Expertise in providing counselling and case management to individuals who are experiencing or have experienced a complex history of mental health concerns, substance use, and involvement in the criminal justice system.
- Working knowledge of best practices in mental health, substance use, harm reduction, trauma and the therapeutic relationship.
- Working knowledge of trauma-informed practice, women centred principles and best practices for women and substance use.
