Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is partnering with LAMP Community Health Centre (LAMP CHC) and Roots Community Services Inc (RootsCS) to design, develop and implement an integrated health and social services hub with and for the Black, African and Caribbean (BAC) communities in Peel Region.

The hub will provide primary health, mental health, addictions and social services programs that will holistically address the physical, mental, social and cultural needs of BAC communities, and seek to eliminate disparities and inequities.

**We are looking for a Community Health Ambassador** **to join our team**

Reporting to the Program Manager, BHSS, the Community Health Ambassador provides culturally specific community outreach, education, presentations, workshops and promotes mental health and addiction awareness and supports within the Peel community and facilitates access to primary care and other community resources.

**Status**: Permanent full-time, 37.5 hours per week, evening hours and weekend may be required

**Set Salary**: $59,468.90 (We have recently completed work to increase salaries for this role for improved market alignment. The salary for this role is set.)

**Location**:This position is based out of our Brampton office. Although we operate within a hybrid workplace model, staff are required to have on-site and/or community presence as per program and client needs

**What you will do**
- Organize Mental Health and Wellness information sessions, distribute resources through tabling sessions/events, presentations at local community venues, religious venues and businesses
- Dispel culturally specific myths/misinformation and address stigma through culturally and language specific education and information sessions
- Provide service navigation and facilitate coordinated access to community supports, including primary care, mental health and addiction services, and social supports
- Monitor clients’ mental and physical health and initiate connections to appropriate resources to promote wellness.
- Deliver of low-intensity mental health interventions, including needs assessments, system navigation, and group facilitation
- Develop and initiate relationships with community members based on trust, respect, and honesty
- Consult, advocate and liaise with community resources and agencies to assist clients and families to access necessary services
- Work in collaboration with community partners to promote mental health supports and education
- Identify trends in community engagement and recovery frameworks
- Gather, compile and report on common and systemic barriers to accessing mental and physical health supports
- Maintain appropriate record-keeping/documentation and client files, statistical and outcome measurement tools as required by CMHA Peel Dufferin and funders
