We are looking for self-driven Care Coordinators/ Home Care Marketeers to join our new team. You will play an instrumental role in generating business from an assigned region and interact in the community to promote company services. Primary referral sources include hospitals, physicians, senior retirement communities, skilled nursing facilities, and the community at large.

You will be responsible for identifying opportunities for formal or contractual relationships with physicians, hospitals, skilled nursing facilities, and assisted living facilities, as well as executing territory sales plans in coordination with operational and clinical leadership to ensure extraordinary customer service.

Your safety is our top priority. We will provide you with all the needed Personal Protective Equipment (PPE) and appropriate training, as well as daily infection prevention screening of clients and workers, prior to visits, to help prevent exposures to COVID-19.

Job Responsibilities:

- Responsible for generating referrals for home health care by establishing and maintaining relationships with hospitals, skilled nursing facilities, assisted living facilities, skilled home care agencies, hospice and other service providers.
- Visit hospitals, skilled nursing facilities, assisted living facilities, doctor’s offices, and other referral sources in our service area regularly and follow up with discharge planners when clients get admitted.
- Do the initial assessments, Care plan and monthly follow-ups for the clients referred.
- Provide updates and statuses on admitted clients and potential referrals.
- Assist in obtaining relevant paperwork from physicians and discharge planners on a timely basis.
- Establish, nurture and maintain existing and generate new professional relationships with all referral sources that contribute to the company’s continued growth.
- Regularly attend daytime and evening marketing and networking events to enhance the visibility of our agency in the community and generate new relationships and business opportunities.

Job Requirements:

- At least two years of field sales experience/ care coordination with a proven track record
- An RN or an RPN is an asset
- Strong interpersonal, presentation, and communication skills
- Excellent technology skills include using a smartphone, laptop, the Microsoft Office suite, and a contact relationship management system.

Work Location: Durham Region/ City of Toronto/York Region/ Peel Region/ Halton Region (Any one of these regions)

**Salary**: From $32.00 per hour
