This position is responsible for developing, planning, implementation, and evaluation of therapeutic recreation activities to enhance the social, emotional, physical, intellectual, and spiritual needs of our residents.**Responsibilities**:_
- Responsible for the initial and on-going assessment of each resident.
- Develop strong connections with family and significant others, to assist in assessing residents social, emotional, physical, intellectual, and spiritual needs.
- Observe residents changing behavior to assess social, emotional, physical, and intellectual functioning.
- Implement one to one, small group and large group interventions structured to restore, rehabilitate, maintain, or improve functioning and independence in life activities.
- Develop and provide a recreation calendar, which identifies all the needs of the assigned residents each month.
- Evaluate the success of the program on a quarterly basis, using the outcomes of the care plan, participant feedback and attendance as measuring tools.
- Publicize on-going and special event programs through bulletin boards on floors and elevators, magicinfo information display boards and provide letters to families and friends when applicable.
- Develop, review, and distribute monthly calendars to each resident, while promoting special events of the month.
- Document in progress notes when assessments and changes in resident’s participation occur.
- Communicate with SH team resident recreation needs.
- Provide information to team members about available programs and services.
- Exchange information with team members regarding resident progress and revise plan accordingly.
- Comply with facility/department policy and procedures.
- Maintain inventory of equipment and supplies.
- Work cooperatively with other team members.
- To work within the guidelines of the operating budget.

- **Job requirements**:_
- Completion of the Recreation Facilitator for Older Adults Certificate or equivalent
- Related experience in the provision of therapeutic recreation.
- Related experience working with older adults in LTC setting.
- Related experience working with adults with dementia.
- Good interpersonal, conflict resolution and leadership skills.
- Demonstrated group facilitation skills.
- Excellent computer skills.
- Able to read, write and speak English.
- Able to work as part of a team.
- Good organizational abilities, with a particular emphasis on time management.
- Physically and mentally able to perform all duties of the role.
- Maintains confidentiality in all matters pertaining to residents, staff, and facility operations.
