**The Role**
Clinic360 is currently recruiting a Part-Time Nutritionist to join our high-performance
team. We are seeking an experienced and client-focused professional that can provide
nutritional guidance and counseling to bariatric surgery patients through development,
implementation, and monitoring of dietary recommendations pertaining to surgery.
Provision of clinical nutrition care requires knowledge of principles of weight loss
surgery and the ability to analyze data, assess nutritional status, and formulate nutrition
plans.

**You Will Have The Opportunity To**:
**Patient Care**
- Work closely with patients with morbid obesity issues.
- Recommend appropriate nutrition intervention to the bariatric surgery patient

including but not limited to, protein parameters, iron, vitamin, and calcium levels.
- Proactively motivate patients to make the desired lifestyle changes prior to and

after bariatric surgery.
- Intensively case manage and counsel patients and prepare them for the surgery.
- Case manage patients after the surgery, monitor and record their progress.

**Patient Assessment**
- Screen the medical record, complete, record and track comprehensive nutritional

and dietary assessment based on medical record data and interview with the

patient.
- Track and analyze dietary data to create individualized dietary and nutritional

plan for the patients.
- Set up expectations of the plan for each patient.
- Conduct group counseling sessions for patients, where necessary besides one to

one sessions.

**Patient Education**
- Educates patients based on cultural, religious and ethnic preferences.
- Individualize patient education materials based on the patient's age and

comprehension levels.
- Provide resources and education according to identified needs.
- Evaluates and develops nutrition education materials.
- 1) Performance Improvement - Identifies issues, collects, evaluates and

reports data as assigned.
- Collaborates with physicians, nurses and other allied health staff to provide

patient care.
- Documents nutrition referrals, as needed, to ensure quality nutrition care

throughout the continuum of care.
- Monthly Q&A interactive group sessions with patients via Zoom
- Willing to complete and/or “Craving Change” program
- Documents nutritional care in the medical record, according to established policy,

procedure and standards.

1) Documentation Documents desired outcomes specific to the individual

patient problem(s), diagnosis(es), learning capabilities/limitations, and

progress towards established goals.

**Administrative Responsibilities**
- Assists with development of clinic standards, guidelines, and policies and

procedures.
- Adheres to all regulatory standards.

**Social Media**
- Assist with creation and publish videos pertaining nutrition and diet specific to

bariatric surgery patients.
- Introduce products and foods for bariatric surgery patients that generally help

them achieve and maintain their health goals.
**What You Will Bring**:

- At minimum, completion of a Bachelor's degree in a 4 year Dietitians of Canada

(DC) accredited university food/nutrition program degree/discipline or recognized

equivalent
- Registration with the College of Dietitians of Ontario
- Membership with Dietitians of Canada, preferred
- Experience in a hospital or clinical environment, required
- Previous experience in an outpatient Bariatric Program/clinic preferred
- Complies with the Code of Ethics and Rules of Conduct according to College of

Ontario
- Recognizes and acts within the scope and limitations of the dietitian's role
- Ability to exercise sound clinical judgment and to function in an independent,

self-directed manner
- Flexibility and ability to prioritize and to deal with changes in tasks with short

notice

- Ability to negotiate and advocate for patients and demonstrated teaching ability,

and leadership skills to facilitate meetings
- Ability to adapt to a wide variety of patient populations and knowledge of cultural

foods and diversity
- Excellent verbal, written and interpersonal communication skills
