**Report to: Director of Care and/or General Manager**
**Scope of Position**:
Provides delegated nursing and personal support services as outlined below. At times, supervises hourly health care management department staff in accordance with established standards, policies, and procedures.
**Key Responsibilities**:
- Performs those functions which provide for the basic care of Residents with an acceptable standard of skill, with a sense of responsibility, and respect for privacy;
- Assists in the care process by observing and reporting changes in the Residents' physical and emotional condition. Knows the guidelines of assigned Residents as outlined in the resident care plans. Gives care according to these guidelines and documents results;
- Performs and/or supports healthcare aides with resident personal care needs as well as selected and delegated nursing measures to promote comfort and safety for the Resident in accordance with our standards of resident service and assigned care packages. This can include:
- Providing direct resident care in accordance with approved standards of care such a monitoring a clients’ vital signs, collecting routine lab exams and administering medications;
- May provide personal care such as assisting with clothing, hygiene, mobility, ambulation, transfers and other routine activities of living;
- To observe and report on changes in residents/clients condition to supervisor;
- Appropriately responds to medical emergencies as per policies. Escalate medical emergencies to supervisor or emergency medical services as appropriate;
- Dispenses medications as per delegation and follows all processes and protocols regarding documentation and medication dispensing;
- Assists with the maintenance of records reflecting each Residents' condition, care and specific treatments. Documents on daily flow sheets, progress notes and care plans as per system;
- Maintains effective communication and good human relations with residents, families and colleagues, using the appropriate lines of communication.(Works co-operatively with families, including them in Resident care needs);
- Assists in maintaining a safe, neat, clean environment for Resident and personnel;
- Shared responsibility for the care of the equipment in the residence and the personal belongings of the Residents as assigned;
- Directs all inquiries regarding Residents' service and conditions to the lead staff or Manager of Health & Wellness;
- Assists in the training, guiding, and supporting of new staff through the orientation period;
- Recognizes safety hazards and the importance of providing a safe environment;
- Assists in maintaining a safe environment for Residents and staff by following safety policies and procedure and reporting hazardous situations in the appropriate manner performs pre-start-up inspections as required;
- Attends in-service education in order to keep informed of current trends in health services and IPAC practice. Participates in residence learning needs assessment;
- Maintains a good attendance record according to policy of the residence;
- Is knowledgeable of the Resident Rights, supports and promotes same;
- Is knowledgeable of RHRA standards and guidelines;
- When no managers are present during evening and overnight hours, the UCP is then the charge staff on duty and will be responsible for absence tracking and calling staff replacements.
- The Wellness team will support the dietary department with serving assistance as required.
- Assists the PSW/Guest attendants as required
- Any other related duties as assigned, or evidently required
**Other**:
- Is responsible for their own actions.
- Is responsible for maintaining and upgrading competence related to the clinical practice and professional responsibility.
- Is aware of employees and supervisor’s responsibilities and follows all health and safety policies and procedures;
- Works safely to reduce the risk of injury to self, co-workers, guests and residents;
- Is alert to, remedies, and promptly reports all actual or potentially hazardous situations.
- All other related duties as assigned
**Working Conditions**
- Prolonged standing and walking;
- Lifting and moving of residents, equipment and supplies;
- Repeated bending and stooping ;
- Exposure to Resident elements through regular Resident contact;
- Stress resulting from dealing with Resident care needs and pressures;
- Some use of visual and auditory sense in monitoring conditions of Residents during care.
**Physical Requirements**
- Lifting alone, maximum 50 lbs. (5-25 lbs. Usual);
- Lifting with assistance, maximum 160 lbs. (100-125 lbs. Usual)as a minor job function;
- Pushing/pulling, using wheeled equipment such as carts, trolleys with contents that weigh a maximum of 100 lbs. (20-50 lbs. Usual) as a minor activity;
- Stretching/reaching on a regular frequency throughout the shift as an essential job function.
