**About the Opportunity**:
Under the direction of the Manager, Policy and Planning with the Primary Health Care - Chronic Disease Management Network (Clinical Excellence and Diabetes Care Program portfolio, this individual provides oversight of individuals and committees/working groups, and leads specific initiatives based on content and context expertise in work across the age continuum to improve health and system outcomes. This includes pediatric, maternal, and adult care, in the various system sectors— primary care, acute care, and long-term/continuing care, where applicable. This position assists in the development and implementation of practice supports (processes, standard protocols, policies, and guidelines) to advance diabetes care in the province. This includes monitoring trends in the diabetes care and research. This is a consultant position, not a direct patient care position. This position will work closely with stakeholders both internal and external to the health system and will lead/co-lead specific initiatives.

**About You**:
We would love to hear from you if you have the following:

- Baccalaureate degree in a Health Profession (Nursing or Nutrition) from a recognized University
- Master's degree considered an asset
- Certified diabetes educator (CDE) required
- A minimum of five years teaching and clinical experience with a strong background in the knowledge and practice of adult learning principles, the chronic care model, program evaluation, and outcome measures
- Must have diabetes education/care/treatment experience
- Familiar with health care redesign, NSH management zones, and the community health boards in Nova Scotia
- Knowledge of quality improvement theory and practice
- Computer and related software knowledge
- Considerable proficiency using Power Point, Word, Audiovisual equipment and teaching resources
- Excellent communication and analytical skills
- Experience writing reports and other publications
- Demonstrated project management skills
- Able to support concurrent projects as lead or contributor consulting internally and with stakeholders throughout Nova Scotia
- Takes a keen interest in maintaining a proficiency in the changing concepts related to diabetes care, chronic disease management, and patient/adult education
- Able to make independent decisions and solve problems using a collaborative, evidence-based approach
- Valid Nova Scotia driver's license with access to a reliable vehicle as travel throughout the province is required
- Demonstrates an interest in continuing professional growth
- Competencies in other languages an asset, French preferred

**Hours**:

- Full time long assignment; 75 hours biweekly
- Approximately 12 months
- Shifts may include some evenings, weekends and holidays
- This is a hybrid work from home position

**Compensation and Incentives**:
$40.15 - $50.18 Hourly
