West Park Healthcare Centre helps patients get their lives back by providing inpatient, outpatient and outreach services in specialized rehabilitative and complex care after a life-altering illness or injury such as lung disease, amputation, stroke, and traumatic musculoskeletal injuries. By 2023, West Park’s 27-acre site will be transformed into an integrated campus of care, including a new hospital building and additional community supports such as assisted living for persons with disabilities and seniors. This transformation enables the Centre to evolve its rehabilitative programs and strengthen links to community-based services to meet the future healthcare needs of our local community and the province.

***Accountabilities**:

- Assess, manage and treat clients/workers based on regulatory requirements and within expectations set out by the Service Level Agreements with customers and service contracts (Ex: the Ontario Workers Network).
- Customer and client centered approach to deliver excellent service, both clinically and administratively.
- Complete timely comprehensive reports summarizing assessment and treatment findings utilizing excellent written communication skills.
- Provide treatment as part of an interdisciplinary team and coordinate interdisciplinary treatment services, reports and communications with assessing teams as needed.
- Coordination of clinical services within the interprofessional team, inclusive of adhering to comprehensive reporting requirements within Service Level Agreements, and external medical professionals.
- Review and interpret reports from Program, Partner, and Community clinicians to provide excellent customer and client centered services.
- Liaise with third party funders and community health care professionals to coordinate care and return to work initiatives in a timely manner, ensuring client/workers smooth transition to the workplace.
- Participation in the collection and analysis of business benchmarks and clinical outcome measures.
- Participate in program planning, continuous quality improvement and evaluation.
- Participate in interprofessional rounds as necessary and relevant.
- Complete timely and thorough clinical documentation.
- Participate in service planning and development of advances in practice, as appropriate.
- Be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors including:

- Compliance with the safety standards of the hospital and applicable legislation
- Identification and prevention of safety issues and problems
- Taking corrective action, where possible
- Ability to work in a manner that exemplifies the Centre’s core values and helps patients_ get their life back._

**Qualifications**:

- Master’s Degree in one of Occupational Therapy, Physiotherapy, or Chiropractic health discipline
- Registration with appropriate health professionals College
- Certified Functional Capacity Evaluator under any system (Metriks, Workwell, Matheson, etc. )
- Cognitive-Behavioral therapy training/certification
- Minimum of 4 years as a practising regulated healthcare professional
- Experience in third-party funded healthcare services requirements
- Demonstrated ability to exercise initiative and good judgment and ability to multi-task
- Strong interpersonal, communication (both written and verbal) and organizational skills
- Ability to be self-directed and effectively problem-solve
- Ability to work effectively within an interprofessional team
- Excellent ability to draft medical reports
- Proficient in the use of Microsoft Word and Excel
- Ability to work (learn) within various IT platforms
- Ability to work flexible hours
- Ability to demonstrate a customer service philosophy
- Evidence of satisfactory attendance and punctuality
- Experience with quality assurance and report review
- Ability to function in a consult model, as appropriate
- Relevant experience in assessing and treating patients with a focus on Mild to Moderate Traumatic Brain Injury including Concussions preferred
- Relevant experience in the treatment of clients/patients with work related injuries, MVA and/or experience in the third party health care system preferred
