Your Opportunity:
The Psychosocial Rehabilitation Program at the Centennial Centre for Mental Health and Brain Injury provides comprehensive recovery orientated services to individuals 18-64 years of age who are living with the effects of mental illness. Our unique program is 8 weeks and is primarily a group’s-based program. We strive to deliver evidence based, goal directed patient and family centered care, which are in line with core competencies of PSR Canada. Reporting to the unit manager, the role of the Social Worker II is to work collaboratively within an interdisciplinary team to provide excellent Patient and Family Centered Care utilizing the core values of AHS. Your role will be to facilitate comprehensive psychosocial assessment, assist the clients and families with achieving their recovery goals, provide patient and family support and education, facilitate groups, lead family meetings, provide crisis intervention, provide client/family advocacy as needed, and engage clients in supportive counselling as required. You will also play a key role in facilitating successful community reintegration by working closely with the patient, family/supports, treatment team and community partners to identify and secure appropriate community resources and supports to ensure a successful discharge.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

**Required Qualifications**:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
**Additional Required Qualifications**:
Excellent communication, organization and interpersonal skills as well as the ability to work in a psychiatric environment.
**Preferred Qualifications**:
Experience facilitating groups. Experience working with families. 3-5 years of mental health experience. Authorization to perform Psychosocial interventions is an asset.
