Hepatitis C Outreach Worker

Reporting to the Health Outreach Coordinator, the Hep C Outreach Worker is responsible for supporting clients who require extra assistance to navigate hepatitis C treatment. The Hep C Outreach Worker will work collaboratively with the Health and Support Services team at the Cool Aid Community Health Centre (CACHC) to coordinate hepatitis C treatment starts and continued adherence to treatment in outreach settings.
A narrow selection of the larger scope and responsibilities for this position are as follows:
- Coordinate and advocate on behalf of clients to start and complete their 8- or 12-week hepatitis C treatment.
- Provide outreach visits to clients at supportive housing sites, encampments, shelters, and support services to engage clients in hepatitis C treatment adherence and support.
- Working closely with local pharmacies, pharmacy staff and community partners to ensure client’s easy access to hepatitis C treatment.
- Provide relevant collateral information to the CACHC team.
- Organize work, set objectives, and establish priorities. Manage time and resources to effectively locate and support clients.
- Work effectively in an interdisciplinary healthcare setting, promoting cooperation and communication.
- Participate in and help prepare for point of care peer testing events in community settings (i.e. shelters, supportive housing sites, food banks, and other support services).
- Integrate best practice to support work with strong competency in harm reduction, trauma informed practices and evidence-based approaches.
- Maintain firm boundaries and confidentiality when working with vulnerable populations who are living with chronic mental health and substance use challenges, those accessing community health services, those living with HIV and Hepatitis C.
- Provide care to Indigenous clients in a culturally sensitive and trauma informed manner.
- Maintain client records by charting client information via electronic medical records and documenting client interactions to ensure continuity of care.
**QUALIFICATIONS**
**Experience**
- A minimum of one (1) year of outreach experience.
- Experience working with client populations facing multiple barriers including infectious disease such as HIV and hepatitis C, physical and mental health challenges, substance use, poverty, and homelessness and other barriers.
**Education**
- Baccalaureate Degree or 2-year diploma in a health-related discipline from an approved post-secondary institution, or an equivalent combination of education, training, professional and lived experience.
**Requirements**:
- Complete the Ministry of Public Safety and Solicitor General Criminal Record Check.
- Class 5 driver’s license and clean driving record.

- **Job Type **Part time - Temporary. This position is expected to last until the end of December 2024.
