**Description**:
Coordinate an entire treatment program for their guest by assisting their guests or residence in more practical terms, like connecting a client with appropriate care organizations and helping their guests and residence navigate difficult life circumstances and focus on improving the quality of their personal life.

**Objectives & Known Results (OKR’s)**:

- Weekly one on ones with each guests or residence of case load.
- Conducting or supporting case workers initial comprehensive assessments for guests or residence needs and support systems. (SPDAT).
- Evaluating needs and provide a personalized plan for each guest or resident.
- Providing proper education on risk prevention to the patients, their families, and support groups.
- Support clients’ compliance with medication and care plans. Helping clients make informed decisions by acting as advocates regarding treatment options.
- Promoting cost-effective and quality intervention programs.
- Support financial, housing and food security with guests and residence.

**Key Performance Indicators**:

- 85% of guests or residents have a SPDAT after 30 days.
- 100% guests have a personalized action plan after 60 days.
- all updates and interactions with guests or residents’ information is in Hifis.

Responsibilities & Expectations:
Developing initial intake report (SPDAT) and care plans, ensure the care plans are implemented, and working connecting with other health workers to provide appropriate care for the needs of each individual.

**Salary**: From $20.00 per hour

Expected hours: 40 per week

Ability to commute/relocate:

- Moncton, NB E1C 6B5: reliably commute or plan to relocate before starting work (required)

**Education**:

- AEC / DEP or Skilled Trade Certificate (preferred)
