Spruce Grove, Canada WestView Primary Care Network Full time

**Purpose**

The WestView Primary Care Network (PCN) and Parkland School Division, in collaboration with CHANGE Health Alberta, are partnering to create a PCN Youth Mental Health Clinic co-located within Parkland School Division's two high schools and two outreach sites.The Social Worker is a key role in this newly initiated clinic. This position supports and coordinates the work of the two participating physicians and other multi-disciplinary team members in addressing the mental health needs of youth in these schools as well as providing social services and navigation to community services. The Social Worker also plays an essential role in coordinating efforts between the school clinic and the Youth Mental Health Clinic within the PCN and will work closely with the corresponding Social Worker in the PCN clinic. This position is not expected to provide therapy services to the youth, however there is an expectation that this position will be able to create safety plans and deal with issues of an urgent manner.

**Key Responsibilities**

The Youth Social Worker role is expected to be flexible and accommodating to the needs of the individual schools and the school district.
- To work collaboratively in a youth centered, integrated team based care delivery system
- To participate in the care of individual patients
- Read and assess referrals for approval or to escalate them to a higher level of care
- Complete intake assessments with youth who present to the clinic
- Complete referrals to outside sources for services
- Provide social services and connections for youth
- Complete letters for court and/or income support
- Organize and run team conferences after each clinic
- Create risk assessments and safety plans for those youth who present in crisis
- Develop and communicate brief intervention strategies with referral sources
- To act as a resource and liaison to family physicians and other multidisciplinary team members, healthcare institutions and community agencies or programs; contributing to service delivery planning including clinical practice and program development, evaluation, and stakeholder consultation
- Maintains excellent working relationships with other local mental health care providers to ensure appropriate referrals
- To ensure accurate, objective and timely clinical documentation in the appropriate medical record for both clinical and evaluation purposes
- Other duties and responsibilities as they emerge; flexible work schedule to meet urgent business needs and/or service requirements
- Willingness to support the vision and mission of the Parkland School District and the WestView PCN

**Position Requirements**
- Bachelors degree in Social Work with ACSW registration
- Training in Suicide Prevention and/or Crisis Intervention
- Experience working with youth in a professional manner
- Knowledge of Province of Alberta privacy laws
- Professional, accountable, excellent written and verbal skills.

Pay: $36.00-$42.00 per hour
