Right at Home is looking for someone to join our team of exceptional caregivers as a Junior Care Planner/ Team Lead who can support community engagement, assist with Care Plan risk assessments for clients and support a team of caregivers and work alongside them by providing PSW/CCA/HCA support for Monday through Sunday in the **Halifax Region**. Must have a valid driver’s license and access to a personal vehicle. Every second weekend availability is a must, open to on-call shifts. References required.

**Responsibilities as Junior Care Planner and Team Lead**:

- Accountable for the day-to-day leadership of CCA/PSW team in collaboration with Care Planner and Care Coordinator.
- Coordination of client care, supervision and support of the care team and problem solving with clients and families.
- Working closely with other caregivers and clients and their families in the community and in retirement homes in a professional, compassionate and meaningful manner.
- Communicating closely with the team and relaying any pertinent information regarding team members and client care needs and/or concerns.
- Relationship development with caregivers, clients and their families.
- Drop off PPE when needed, assist with recruitment, etc.
- Be able to assist with Business Development
- Participate in the initial screening, hiring and supervision of caregiving staff
- Engage directly with the Right at Home management team, business development representatives, and other staff as an integral part of the company’s goal to continue to be Canada’s leader in the delivery of family-funded home health care services
- Assist in achieving an exceptional experience for the Caregivers within our Right at Home system through warm, positive communication, encouragement, inclusiveness and leadership.
- Other administrative duties as assigned, as well as office upkeep and filing

**Essential Functions - Direct and Delegated Responsibilities**
- Conducting the client assessment time to time
- Acting as a resource to client families
- Spearheading the Care Team assembly
- Help Coordinating the care and schedule with their Scheduler
- Responding to scheduling and client care urgencies
- Supervising, managing and motivating the Care Team members
- Travel across the Halifax region is an expected and anticipated part of your role. Conducting meetings, consultations, and engagements will happen anywhere within the broad region.
- Drive some marketing strategy, social media strategy, business development, business growth and other communication channel strategy for brand and services promotion.
- Schedule and conduct meetings with various referral sources to provide awareness of, and promote, Right at Home supportive care services, based on the classroom and field training.
- Conduct exceptional follow-up to your referral source meetings and meaningfully grow those relationships.
- Monitor and attend relevant community events for community-awareness opportunities.
- Be an excellent brand ambassador for the Right at Home name.
