- Le CIUSSS de l’Ouest-de-l’Île-de-Montréal est présentement à la recherche de techniciens ou techniciennes en travail social pour combler des besoins à temps complet de jour au Centre de réadaptation de l’Ouest de Montréal (CROM). Les personnes sélectionnée seront appelées à travailler auprès d’une clientèle atteinte de déficience intellectuelle, de troubles du spectre de l’autisme ou de déficience physique (DI-TSA-DP) et hébergée en ressource intermédiaire (RI) ou ressource de type familial (RTF).
- Dans le cadre de vos fonctions, vous serez appelé(e) à vous déplacer sur un territoire dynamique, couvrant près du tiers de l’île de Montréal. Bien sûr, les frais de déplacement (kilométrage) sont remboursés.
- Le CROM est une installation du CIUSSS de l’Ouest-de-l’Île-de-Montréal qui offre des services de réadaptation en vertu des orientations de la Direction des programmes de DI-TSA-DP. Il a pour mission de favoriser l’autonomie et la qualité de vie des usagers, et fournit des services à des usagers de tous âges ainsi qu’à leurs familles.

- Dans le cadre de vos fonctions comme technicien(ne) en travail social, secteur adulte et enfance, vous serez appelé(e) à:
- Assurer le suivi de l'usager, dans sa ressource, en collaboration avec l'équipe de professionnels en place, en vue de l'adaptation ou de la réadaptation;
- Observer et analyser le comportement des usagers, participer à l’évaluation de leurs besoins et de leurs capacités, documenter la progression et définir leurs programmes d’activités;
- Assurer l'encadrement des usagers et la gestion de crises tout apportant un soutien émotionnel;
- Assurer la promotion et la défense des intérêts et des droits de l’usager et de ses proches;
- Procéder à l'évaluation/réévaluation des besoins de l'usager tout au long de son hébergement;
- Déterminer, avec l’usager et son réseau, les objectifs de réadaptation et identifier les moyens pour y répondre, le tout dans le contexte d’un plan d’intervention;
- Accompagner, soutenir et référer l’usager et son réseau aux services auxquels ils sont admissibles tout en prônant l’autonomie de la personne et de son entourage dans la démarche;
- Émettre des recommandations cliniques et les communiquer à l’usager, à son réseau et aux intervenants concernés;
- Soutenir l’intégration (admission) et la post-intégration de l’usager dans la ressource;
- Travailler au sein d’une équipe multidisciplinaire.

- Horaire de travail : lundi au vendredi, 8h30 à 17h ou 9h à 17h30 - Exceptionnellement, la personne retenue pourrait être appelée à travailler en dehors de ces heures, de manière prévue d'avance, et avec horaire aménagé en conséquence.- Requirements :- Détient un diplôme d'études collégiales (DEC) en techniques de travail social ou en service social;
- Est apte à établir une relation thérapeutique avec une clientèle francophone et anglophone;
- Possède un permis de conduire valide et une voiture pour les déplacements (kilométrage remboursé).
