**Title**:_ YTU (Youth Transition Unit) Caseworker_

**Reports to**: _Director of YTU_

**Based at**: _Head Office; Community_

**Employment Type**:_Permanent Part Time (12-24 hours/week)_

**Pay Range**: _$16.00 - $20.00 based on education & experience_

**Hours**: _Afternoons & evenings, weekends optional_

**Description**

As a YTU Caseworker you will provide 1:1 support to a caseload of 4-5 youths, ages 16-21, as they work to build the life skills, healthy relationships, and community connections necessary to successfully transition to independence. You will identify gaps in resources and barriers to success then develop and implement case plans to support the goals that align with each participant’s unique vision of success. YTU Caseworkers are expected to comply with industry standards and best practices while adhering to all SCP mandated approaches, policies, processes, and protocols in the fulfillment of their role and completion of their duties.

**Qualifications (Education and Experience)**
- Secondary completion required (High school diploma or equivalent)
- Human services degree/diploma/certificate strongly preferred
- 1 year human services, front line youth support, youth care experience
- Preference will be given to applicants with experience in:

- Community networking, non-violent/needs based communication, design & delivery of life skills programming, administrative duties
- Preference will be given to applicants with experience supporting:

- Individuals experiencing current/previous exploitation, individuals managing substance use and/or mental health concerns, & individuals with gang involvement
- Vulnerable & Indigenous populations
- Preference will be given to applicants with experience working with:

- Ministry of Social Services, Child & Family Services, & Community Resources

**Requirements (Skills & Competencies)**- **Valid Driver’s License with Driver's Abstract**:

- **Criminal Record Check **with** Vulnerable Sector Clearance**- **Cultural awareness required**
- Self directing with strong initiative; intrinsically motivated
- Strong communication skills & precise documentation
- Strongly developed interpersonal skills, emotional intelligence
- Well developed time management, organizational skills, & attention to detail
- Preference will be given to applicants with:

- CPR/First-Aid (B), Conflict Resolution, Crisis Intervention (DPVS), ASIST (Suicide Intervention), Mental Health First Aid & Water Safety
- Demonstrated mastery of official SCP sanctioned approaches (Nurtured Heart Approach, Youth Centered Care, Harm Reduction, & Trauma Informed Care)

**Duties & Responsibilities (Outcomes to be monitored quarterly)**

**Youth Support & Life Skill Development**
- Identify essential life skills that require development/support
- Activities of daily living (hygiene, cleaning, cooking, meal planning, etc.)
- Budgeting and financial literacy skills
- Transportation, navigating public transit
- Accessing education, health care, & community resources
- Obtaining and maintaining employment
- Healthy relationships and boundaries
- Mental health and substance use management
- Safety planning
- Facilitate connection to resources that teach/support life skills
- Model healthy and well developed life skills
- Advocate for youth (i.e education, family, medical, mental health, substance use)

**Case Management**
- Develop, implement, oversee, review, & update participant case plans
- Identify and bridge/support resource gaps
- Identify all relevant community/cultural/peer resources
- Facilitate healthy community engagement that supports participants goals/interests
- Complete detailed and objective case notes in a timely fashion
- Assist participants in obtaining relevant ID/documents (i.e photo ID, health card, treaty card)

**Program Development**
- Attend meetings as required
- Comply with agency, program, and funder reporting requirements
- Maintain a working knowledge of/ensure compliance with
- Current SCP policies, procedures, guidelines & emergency protocols
- Best practices (progressive evidence based approaches)
- Other tasks as assigned
