In keeping with the philosophy and objectives of the Behavioural Supports Ontario model and in collaboration with internal and external partners in care, the HNHB LTCH BSO Behavioural Lead will use a team approach to provide holistic care to the residents of LTCH’s to which they or their team are referred.

The Behavioural Lead will operate within the Scope of Practice as outlined by their registering body; the Ontario College of Social Workers and Social Service Workers.

The BL as a supervisory team member, who demonstrates person centered care approaches emphasized through the completion of assessments, development of care strategies, modelling successful strategies and interventions to support the LTCH to manage and care for residents with responsive behaviours.

**Duties and Responsibilities**:

- Social Service Worker/BL will work collaboratively with care partners and consultants serving Long Term Care homes. This includes Alzheimer Society PRCs, Specialized Geriatric Services, Palliative Care and Pain Consultants, Community BSO services, LTCH BSO Mobile, BSO Transitional Leads and discharge planners, Community Support Service agencies and other health service providers as required
- Utilizes and completes approved assessment tools as per established BSO practice and scope of practice; Completes required/ identified assessments and documentation based on evidence based best practice, BSO practices and College Standards.
- Works with the specific LTCH leadership team to support the needs of the individual and with consent from the resident reports on the identified strategies, plan of care, assessment and results.
- Participates in Continuous Quality Improvement practices as identified by the BSO Program, its Director and/or Oversight Committee through data collection, analysis, improvement initiatives and PDSA cycles as directed/ requested
- Supports residents with Independent Activities of Daily Living by: implementing interviewing skills such as: building the therapeutic relationship, creating a safe and welcoming space, active listening, asking open-ended questions, cognitive reframing and paraphrasing resident/s conversation to draw connections that might not seem obvious to the resident.
- Provide recommendations on bio-psychosocial and non-pharmacological interventions to support resident goals in collaboration with the resident/ their family / significant other and the interdisciplinary care team. Advocates for and protects resident rights and self-determination by facilitating education or consultation to LTCH staff
- **Supervisory Duties**:

- Supervises the practice, accountability, performance and conduct of the BSO Mobile CSW (Care Support Worker) assigned to the targeted Behavioural Lead home(s).
- Participates in the interview and selection process of BSO Care Support Workers assigned to the targeted homes.
- Completes the probationary, annual or q2 year performance appraisal of CSW’s assigned to them

**Knowledge, Skills, and Experience**:

- Diploma in Social Services in good standing and must maintain College Registration at all times
- At least 2 years’ experience in Long Term Care, specifically behavioural units, working within a Mental Health Setting or setting providing services to older adults with responsive behaviours related to dementia, mental health, addictions or other factors.
- Training in Montessori, GPA, adult mental health is an asset
- Demonstrated leadership and ability in coaching, teaching, mentoring staff
- Ability to adapt positively and respectfully to changes in work environment and work location
- Valid Drivers’ License and reliable vehicle with two million liability required
