Rehabilitation Support Worker

Responsibilities:
- Provide support and assistance to individuals in their daily activities
and returning them to an optimal level of independence.
- implement cognitive, physical and /or behavioral interventions.
- work with adolescent, adult and senior population
- work with various ethnic groups and respect all cultural traditions
- provide support in a variety of settings including Rehabilitation facilities and programs ad well hospital, homes and workplace and throughout the community.
- Follow care plans and communicate effectively with healthcare professionals.
Work under the direction and supervision of a regulated health care professional such as Occupational therapist, physiotherapy, speech language therapy, etc.
Liason with all the members of treatment team to ensure effective service provision.
Prepare and submit detailed progress notes and comprehensive progress reports
**Skills**:
- Strong communication skills to effectively interact with individuals, families, and healthcare professionals
- Compassionate and patient demeanor when providing care and support
- Ability to prioritize tasks and manage time effectively
- Attention to detail in documenting observations and reporting any changes in condition
**Job Type**: Part-time
Pay: From $25.00 per hour
Expected hours: 20 - 25 per week
