**KEY FUNCTION**:
Lead and improve system-level processes for discharge planning of all adult acute care at HSN. Lead and prioritize improvement work to enable improvement of the system **related to Alternative Level of Care (ALC) prevention and avoidance. **Provide leadership and direction through planning and organizing of the day-to-day operations of the Amberwood alternative hospital site and promote the delivery of optimal care/service to patients through management of human and material resources.

**REPORTING**:
Under the general direction of the Administrative Director, Care Transitions, Patient Flow and Utilization.

**DUTIES**:

- Work with HSN teams, Patient and Family Advisors, and HSN’s care partners, to identify and prioritize opportunities to further design, develop and/or improve future state processes and standards of work related to discharge planning and ALC avoidance.
- Coordinate the activities of improvement teams, identify appropriate resources needed, and develop schedules to ensure timely completion of improvement work.
- Identify opportunities for improving the transitions from HSN to other care facilities/ agencies through collaborative discharge planning.
- Work with the ALC Prevention Committee to identify opportunities for ALC avoidance, and lead the implementation of prioritized initiatives.
- Monitor the day-to-day activities within the discharge planning value stream to ensure that improvements are being made.
- Measure the impact (value creation, waste reduction) of improvements as the system moves to the future state value stream and report key performance indicators to stakeholders.
- Analyze data to understand emerging trends and recommend resource allocation to address shifts in the presenting population(s).
- Promote early identification of barriers to achieving improvement outcomes, and engage in four step problem-solving at the unit, program, organizational and system level.
- Ensure that all improvement work is patient-focused, and engages patients and families as key partners in system re-design; evaluate patient care and satisfaction and formulate solutions for improvement.
- Champion the adoption of evidence-informed discharge planning and best practices related to ALC prevention.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in the accreditation process and work to ensure that the program and HSN achieve, maintain and continually
- Work collaboratively with Access and Flow Management to help manage day-to-day activities and patient flow within the program.
- Design, promote, and facilitate the use of systems to ensure that the provision of patient care is consistent with program and professional standards.
- Teach, coach and lead staff in the development and monitoring of quality indicators and improvement initiatives.
- Develop, coordinate, and maintain staff schedule/rotation for Amberwood-Alternative Hospital Site.
- Ensure program delivery is consistent with regulations, legislation, professional practice, and evidence-based best practices.
- Engage in and foster professional development of all staff and provide opportunities for learning within budgeted resources.
- Ensure a safe environment for patients, staff and visitors; investigate report, debrief and take or direct corrective action as required on incidents.
- Collaborate with internal and external stakeholders/organizations/community groups to achieve program objectives, integrate and link services and foster partnerships across the continuum of service delivery.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a Bachelor’s Degree with a specialization in a health-related field from an accredited university.
- Advanced training in lean management and continuous quality improvement is preferred.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- Minimum of five (5) years’ experience working in an acute care environment.
- Minimum of two (2) years’ working in a leadership/supervisory or process improvement role.
- Experience working with multidisciplinary teams, physicians, patients and families to improve outcomes.
