**Job Responsibilities (may include but are not limited to)**:
Providing Intensive home, school and community based assessment and treatment intervention for children in Brantford, ages 0 - 18. These clients have been identified as having complex needs which may include mental health challenges, dual diagnosis and may also be developmentally delayed. Incumbent will be involved in coordinating and collaborating with community partners and in developing individualized goal-focused treatment plans. Incumbent must be able to work flexible hours, including evenings and weekends.

**Qualifications**:
***:

- Must have Child & Youth Worker (CYW) diploma, or degree/diploma in related field
- Minimum 3 years’ experience in a related field and experience working with children/youth with complex needs
- Experience working with families and ability to work from a strength-based, collaborative perspective
- Experience working with developmentally delayed children and youth
- Must be able to coordinate and facilitate meetings with community partners
- Ability to work independently and as part of a multidisciplinary team
- Ability to work flexible hours and strong skills in assessment and crisis intervention
- Valid driver’s license and daily use of a reliable vehicle with the ability to transport clients
- Computer skills
- Current Vulnerable Sector police check
