New Path Youth and Family Services is an accredited children’s mental health agency providing a range of compassionate, innovative and accessible services for children, youth and their families throughout Simcoe County. **PURPOSE OF POSTION**:
To provide evidence based/informed individual, family, and group counselling services to youth in home, school and community settings and develop working partnerships with designated schools.

**JOB DUTIES AND RESPONSIBILITIES**:

- Assess the needs of youth formulating treatment plans; including developing goals and service direction.
- Meet with youth, and their family when appropriate, to provide support via short-term and long-term counselling using evidence based/informed programs.
- Actively participate in the treatment planning process including monitoring of each client’s progress towards the attainment of their identified goals.
- Initiate and/or attend case conferences meetings as needed.
- Provide follow-up service to discharged clients as needed.
- Assist clients in making connections to community resources, including making referrals to other ongoing therapy programs, when required.
- Act as the case manager for assigned clients.
- Provide consultation to New Path staff, community partners or other agency personnel about service and support offerings available in the AO program..
- Promote and educate school personnel about New Path services.

**QUALIFICATIONS**:

- Bachelor of Social Work Degree
- Minimum 2 years’ experience working with children, youth and their families in a
mental health agency or related service.
- Experience working with evidence based/informed programs

- Competency and effective use of databases and Microsoft Office systems.
- Confident, articulate and professional when communicating verbally and in writing.
- Strong clinical problem solving and crisis management skills.
- Demonstrated ability to provide individual, family and group counselling.
- Strong case management and organizational skills.
- Strong assessment skills including risk management.
- French language is an asset

**WORKING CONDITIONS/PHYSICAL REQUIREMENTS**:

- Must be available to provide 4 hours per week of direct service to clients after 5 p.m. (evening appointments) to accommodate client and family schedules.
- Willingness to travel to other agency offices and the community using personal vehicle.
- Must have valid driver’s license with minimum G2.
- Prolonged periods of sitting and working on a computer.
