**Shifts: Days (0.5 FTE)**
Reporting to the Manager - Regional Rehabilitative Care Program (RRCP), the Social Worker working with the Community-based Stroke Outreach Program will provide services (in person and virtually) within a regional outreach service specific to clients with stroke and those with chronic diseases. The Community-based Stroke Outreach Program is a pilot service designed to:
- Provide access to community-based stroke rehabilitation for the region East of Thunder Bay (ie. Nipigon, Terrace Bay, Manitouwadge, etc) and to remote, fly-in First Nations
- Services will be both in person in the community the client lives as well virtual. In person will be a combination of outpatient, partnering with the local hospital/First Nation Home and Community Care, and providing services in the client's home.
The Social Worker provides services to clients and/or their Substitute Decision Maker (SDM), family members, and other support persons in the client's life when applicable. The Social Worker is responsible for providing comprehensive assessment and intervention that addresses the psychosocial needs of the clients and/or others. The Social Worker is an integral part of the interprofessional team, providing a social work perspective and acting as a liaison and/or consultant to internal and external programs within the scope of practice. Identifies opportunities for and participates in quality improvement activity within the care setting.
The MSW level position requires autonomy, leadership and specialization in practice.
**Qualifications**:
- Education/Experience:
- Master of Social Work degree (MSW) with training and/or certification in a specialty area preferred (e.g. stroke, chronic disease, rehabilitation, etc.)
- Current registration with the Ontario College of Social Workers and Social Service Workers
- Thorough knowledge of the Code of Ethics and Standards of Practice as set out by the Ontario College of Social Workers and Social Service Workers
- Comprehensive understanding of the social determinants of health of Indigenous people and the social consequences and risk factors associated with client conditions (i.e. addictions, trauma, mental health )
- Experience related to client care for people from Indigenous, stroke, and addictions and mental health populations
- Training and experience in motivational interviewing and solution focused methods, principles in self-management and strengths-based interventions
- Knowledge and experience in establishing networks of support and accessing community resources and services.
Skills/Abilities:
- Ability to manage a complex clinical practice in a health care setting
- Understand the importance of trauma informed practice through a cultural lens
- Knowledge of Indigenous cultures and cultural practices, world view, history, historical trauma, intergenerational trauma, history, impact of colonization, and social determinants of health
- Assessment and treatment of complex psychosocial issues
- Knowledge and skills in a range of theoretical perspectives, psychosocial rehabilitation principles, self-management and holistic wellness approaches
- Ability to communicate effectively and concisely, both orally and in writing
- Ability to establish effective and collaborative working relationships with a variety of stakeholders including community partners within the Indigenous community
- Ability to work independently and collaboratively within an interprofessional team environment with skills, knowledge and attitudes in: interprofessional communication; patient/client/family/community-centred care; role clarification; team functioning; collaborative leadership and interprofessional conflict resolution
- Strong time management, organizational skills, analytical/critical thinking, problem-solving, and decision making skills
- Ability to use independent judgement and maintain confidentiality of records, actions, and information
- Demonstrated regular attendance at work
**CONDITIONS OF EMPLOYMENT**
- Will be required to travel to various work locations throughout the District of Thunder Bay (East of Thunder Bay i.e. Nipigon, Geraldton, etc.) and remote First Nations north of Sioux Lookout, and provide own reliable transportation
