**Summary of CHEO Extensive Needs Service**

The Extensive Needs Service is simplifying access to the right care for families of children and youth living with multiple medical, social, physical and developmental healthcare needs. This is an exciting opportunity to better serve children, youth and their families. The program is currently being developed with our partners and families. There isn’t another program like it in Canada.

Families participating in the program connect to a team of professionals who work together to provide tailored support based on the individual needs of the child or youth and their families. Services can include personalized behaviour support plans, mental health assessments and treatment, counselling for parents and caregivers, medication adjustment, and specialized interventions to support children and youth to build skills such as communication and self-regulation.

For each child or youth, admission into this service begins with an integrated medical, behavioural and social assessment followed by (as needed):

- Medication management (psychopharmacology)
- Specialized behaviour services
- Mental health assessment and treatment (psychology, psychiatry)
- Social supports (e.g. social work, including counselling for parents)
- Collaboration with specialized health care clinics
- Respite
- Coordinated service planning
- Transition support (return to school, return to community core services, transition to adult services)

The program aims to serve - children or youth aged 0 to 17 years, with multiple medical, neurodevelopmental, and mental health needs, who have received or are receiving services across multiple sectors but whose needs are still not met, with challenging or interfering behaviours that impact safety (of self or others) and pose a barrier to participation in school or family life.

One of the unique things about this program is the many disciplines working together to deliver the care needed. The team includes the following roles: Behaviour Analyst, Child and Youth Counsellor, Dietitian, Family Resource Worker, Family partners, Community partners, Instructor Therapist, Intake Worker, Mental Health and Addictions RN, Nurse Practitioner, Occupational Therapist, Pediatrician, Patient Service Clerk, Psychiatrist, Psychologist, Respite Worker, Service Planning Coordinator, Speech Language Pathologist, and Social Worker.

**Extensive Needs Service Counsellor Position**

The CHEO Extensive Needs Service (ENS) Counsellor is responsible for providing counselling services offered under the program, including travelling to sites throughout the Champlain region (at times outside of Ottawa) with the potential to serve clients in their place of residence. The counsellor will work collaboratively in conjunction with clients as part of the multi-disciplinary ENS team.

**RESPONSIBILITIES**
- Provide clinical practice and interventions to youth population relating to intake, assessment, education, treatment, consultation, and referral for clients who are currently experiencing, or are affected by problems related to substance use;
- Provide individual counselling, group counselling and psychoeducational groups to clients, both in short-term & long-term capacities, as well as at Rideauwood and in the community;
- Work effectively with clients who present with multiple and complex problems, including challenges with substance use, concurrent addiction and mental health issues, legal problems, unemployment and limited income, medical problems, and limitations imposed by restrictions in access to other socio-economic determinants of health;
- Work within Rideauwood’s framework and standards, using harm reduction or abstinence focused models based on the client’s needs;
- Review and evaluate client’s progress and adjust treatment plans as required. Provide feedback and support to clients;
- Accept and make referrals to outside agencies;
- Take an active role in preparing for and participating in community presentations, activities and meetings;
- Participate in all assigned meetings, staff development and training as required;
- Contribute actively to administrative and teamwork functions;
- Maintain client records and statistics; and,
- Other duties, as required.

**EDUCATION, SKILLS, AND QUALIFICATIONS**
- College diploma (e.g., CYW, SSW) and/or Bachelor’s level University degree with a specialization in behavioural sciences (e.g., psychology, social work, counselling).
- A minimum of 3 years experience working with youth and families who are experiencing issues related to addiction and mental health issues.
- Assessment and treatment planning experience and competency.
- Experience as part of system intake/navigation and knowledge
