**Summary of Job**:
The Social Worker will provide supportive counseling, in person, virtually or via telephone, to individuals age 16 + in a primary care setting. They will refer patients to appropriate community and regional resources. This is an excellent opportunity to work in an interdisciplinary team environment within a Family Health Team model of primary care.

**Qualifications and Responsibilities**:

- Will possess a Masters of Social Work degree
- Vulnerable Sector Check required
- Strong computer skills
- Ability to work well independently
- Collaborate effectively with physicians, psychiatrists and other team members in a shared care model
- Strong interpersonal and communication skills, both written and verbal
- Experience working in a community setting
- Experience working with individuals with complex needs
- Demonstrate sensitivity to diversity, whether related to race, religion, culture, sexual orientation, and/or gender identity
- A valid driver’s license
- $2 million vehicle liability insurance
