The purpose of this position is to provide oversight and coordination to the Waypoint At Home Program. The Care Coordinator acts as a liaison, as well as builds and maintains relationships between Waypoint (as the lead agency), clients/caregivers, community partners, and other relevant services. This position will work to proactively support Waypoint clients to successfully transition from hospital to community where there are complex needs and the client does not meet the eligibility for current health services offered or is awaiting receipt of other services. The main goal is to support stabilization post-hospitalization and optimizing recovery while being connected to community supports.

**KEY ACCOUNTABILITIES**:

- Provide direct client assessment prior to discharge from hospital and in the community, care plan development, engagement of the At Home team, and coordination of services to meet client needs.
- Build capacity among clients, caregivers, health service providers, and other relevant partners through health teaching, education, coaching and mentoring.
- Work collaboratively with clients, caregivers, members of the interprofessional team, other health care providers, and relevant partners to support the needs of the clients and their caregivers and communicate across the health system.
- Engage in caseloads review and wait list management to support timely access to care, person-centred needs, and risk management strategies.
- Engage in development and continuous quality improvement initiatives for the Waypoint At Home Program.
- Facilitate continued engagement and communication with all stakeholders in building and maintaining relationships with community partners and other relevant services/providers.
- Participate in committees and committee work, data collection and analysis, program and project development, continuous quality improvement initiatives, and research as appropriate.

**REQUIREMENTS/QUALIFICATIONS**:

- Undergraduate degree in a health related field required.
- Master’s degree preferred.
- Registration in good standing with an applicable Regulatory college (e.g. College of Nurses of Ontario, Ontario College of Social Workers and Social Service Workers, Ontario Society of Occupational Therapists).
- Experience with integrating services an asset.
- Knowledge of relevant legislation, policies and procedures, including but not limited to the Mental Health Act, Regulated Health Professions Act, Personal Health Information Protection Act, Health Care Consent Act, Connecting Care Act, relevant sections of the Criminal Code of Canada, Collective Agreement, Ministry and hospital policies and guidelines.
- Experience with psychiatric clinical practice, team processes, risk management procedures, professional standards, research activities and current trends and approaches to treatment and rehabilitation of mental illness in order to lead staff and to assess, implement and evaluate effective delivery of services and operational needs within an inter-professional, community base environment.
- Knowledge of program and hospital documentation, policies and procedures in order to practice within the expected standards of the organization and to recognize abnormal events that may affect client care, activities or safety.
- Knowledge and experience with project management, quality improvement and risk management theory, practice and methodologies in order to utilize resources effectively, to continue to develop the Waypoint At Home program and to promote a healthy and safe workplace.
- Results-oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results.
- Demonstrated teamwork, interpersonal, communication and organizational skills.
- Proven written and verbal communication skills.
- Proven ability to adapt readily to change.
- Models and promotes core ethical practice, Waypoint Values, and reflects an optimistic and positive attitude.
- Preference for proficiency in French/English language skills.
