Reporting directly to the Coordinator - Infection Safety, and working in close collaboration with the Executive Director and Director of Care at Mount Hope Centre for Long Term Care, the Infection Control Practitioner (IPAC Lead) has primary responsibility for providing oversight and leadership to the infection, prevention and control (IPAC) program at Mount Hope and secondary responsibility for supporting the IPAC program at St. Joseph's Health Care London. This role provides oversight and leadership to ensure regulatory compliance and implementation of clinical care standards and best practices related to infection prevention and control. The Infection Control Practitioner will ensure that Mount Hope is meeting its obligations in compliance with Long Term Care legislation, St. Joseph?s Health Care London policies, Public Health requirements and standards and IPAC best practices.
- **Responsibilities**:

- Working with the St. Joseph's IPAC team and Mount Hope leadership and interdisciplinary team to implement the infection prevention and control program.
- Overseeing the delivery of infection prevention and control education to all staff, caregivers, volunteers, visitors and residents.
- Conducting regular infectious disease surveillance.
- Planning for and assisting with required improvements to the infection prevention and control program.
- Conduct, provide reports on and summarize recommended corrective actions related to routine audits.
- Leads the Outbreak Management response for the Home
- Ensuring the hand hygiene program is in place and is compliant with organizational policies as well as long-term care directives.
- Monitoring and reporting on home-level clinical performance data quarterly, including RAI data reports; identifying and evaluating trends and outcomes; analyzing variances and resource needs.
- Leads immunization program for residents (i.e. support with determining resident eligibility, providing immunization updates, develop influenza binders, etc.).
- Develops, oversees the implementation and delivers IPAC training and education program for residents, caregivers, staff and visitors.
- Consults and communicates with the Medical Director of the Home, Mount Hope leadership and staff.
- Works closely with the Infection Safety Team and participates in an organizational on call rotation.

**Essential Qualifications**
- Completion of a Degree in Health Sciences, Social Sciences or approved equivalent combination of education and experience. Degree in Nursing, microbiology, epidemiology or public health is an asset. Preference will be given to Registered Nurses or Registered Practical Nurses
- Current registration with relevant College
- Completion of IPAC Canada endorsed course and Certification in Infection Prevention and Control through IPAC Canada required within two (2) years of hire
- Current experience performing activities that are client centered, while incorporating infection prevention principles
- Knowledge of the legislation, guidelines and regulations as they relate to the position and responsibilities of the Long-Term Care.
- Knowledge of and demonstrated ability in analysis, measurement and evaluative methods, facilitation and presentation skills
- Demonstrated experience with best practice guidelines for infectious disease prevention and control
- Demonstrated experience with the promotion and implementation of clinical best practices
- Demonstrated experience with health promotion and health education/teaching activities
- Demonstrated experience in clinical quality improvement and change management.
- Ability to build and maintain effective relationships and partnerships with a diverse range of stakeholders as well as work effectively within inter-professional teams.
- Effective critical thinking, analytical decision-making, and problem-resolution skills and the ability to distill complex information into clear, concise messages.
- Ability to manage competing priorities and demands in a rapidly changing environment.
- Ability to effectively communicate both verbally and in writing, with excellent interpersonal skills.
- Proficient computer skills with word, excel and electronic document software.
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

**Preferred Qualifications**
- Proficiency in French would be an asset
