**Salary: $29.34 - $35.07/HR - 35 Hours per Week**

**Location: Naughton, ON**

**POSITION SCOPE**:
Reporting to the Health Programs Manager, the Outreach Worker will be compassionate and respectful while providing support to at-risk youth and adults who maybe experiencing mental, emotional, spiritual, and physical experiences. Focus will involve engaging individuals affected by substance use through Community Outreach and complemented by in office hours for support and referrals.

**REQUIRED QUALIFICATIONS**:
Education and Experience
- Minimum Post-Secondary Diploma in Social Services or Addictions
- Minimum of two-years work related experience within a First Nation Community

Mandatory Requirements
- CPIC (Vulnerable Sector)
- Valid and clean Driver’s License Class “G”
- Must be able to work overtime and flexible for evenings and weekends

**KEY RESPONSIBILITIES**:

- Prepare and organize patrol materials
- Provide information regarding Atikameksheng programs, services and opportunities
- Ensuring that safety and well-being practices are implemented; this includes being knowledgeable of Universal Care precaution practices, emergency systems and procedures in dealing with crises situations, communicable diseases, duty to report
- Developing linkages and maintaining effective working relationships with other community services/agencies to facilitate contacts and services
- Implement Circle of Care (Case Management Model for Atikameksheng Anishnawbek)
- Coordinate, implement and evaluate the Peer Support Program
- Coordinate Peer Support Training in the community
- Work in collaboration with the internal and external program/services.
- Make referrals to internal/external services as required
- Record statistics and prepare monthly reports
- Input client care plans and updates into “Casework” EMR System
- Develop a workplan
