Detailed Overview:
Provides assessment and treatment services for Continuing Care clients who experience a variety of medical diagnoses; works with clients and families, liaises with community agencies, caregiver support groups and the multi-disciplinary Home Health Care team; participates in educational services, planning, program development, and policy recommendations; provides consultation, support and the social work perspective to the multi-disciplinary team in the development of services within the assigned area of responsibility.

**Responsibilities**:

- Provides therapeutic counselling services to Continuing Care clients and families experiencing complicated circumstances relating to a variety of medical diagnoses.
- Conducts social work and psychosocial assessment of clients and families and interprets the information using a variety of modalities in accordance with professional standards and clinical practices.
- Follows up and provides ongoing monitoring of complex psychosocial situations; evaluates clients' response to interventions.
- As a member of a multi-disciplinary team, provides information regarding clients psychosocial situation and participates in developing a comprehensive plan of care with/for the client; collaborates and consults with other community professionals.
- Acts as a resource for clients and their families by providing information regarding community resources and access to these resources, facilitating referrals where appropriate, and facilitating the development of community resources by identifying services needed to support the health of families.
- Participates in quality improvement activities by providing input into the development, implementation and evaluation of program goals, objectives, policies and procedures.
- Promotes client autonomy by ensuring their participation in planning and decision making, and the development of problem solving abilities.
- Assists in the development and provision of relevant educational programs for Continuing Care staff, clients, their families and community agencies on psychosocial issues related to health, illness and disability.
- Prepares statistics, in accordance with the workload measurement system, for submission to designated personnel; maintains written clinical records.
- Performs other related duties as required.

Qualifications:
**Education and Experience**

Master's Degree in Social Work from an accredited School of Social Work plus one (1) year's recent, related experience.

Current full registration with the BC College of Social Workers. Must maintain a valid B.C. Drivers License.

**Skills and Abilities**
- Comprehensive knowledge and understanding of counselling skills and clinical practice models.
- Demonstrated ability to conduct social and psychosocial assessments.
- Ability to provide leadership to the interdisciplinary team in care planning in complex client situations.
