The mission of the Oxford County Community Health Centre (OCCHC) is to reduce inequities and improve lives by providing a range of health care & support services for those experiencing physical, mental, emotional, economic, social, and other challenges.
- **Housing Stability Worker (Tillsonburg & Ingersoll) **_**April 2023 - March 2024**

**OCCHC is seeking an experienced Housing Stability Worker (HSW) to deliver intensive case management services to** **clients in the Tillsonburg, Ingersoll, and surrounding communities**. The HSW will provide community-based support to individuals who have received an eviction notice or have recently secured new housing but have a history of homelessness or housing instability. The program provides hands-on, low barrier support to prevent evictions in our community. The position involves assistance with Eviction Prevention in partnership with the legal clinic services, as well as budgeting, community support network development and advocacy to ensure their housing stability is a priority. In addition, the position requires extensive knowledge of community resources for referral purposes. Case Management includes: (1) assessment of the participant to determine service needs; (2) development of an individualized support plan; (3) referrals and related activities to help the participant obtain needed services; (4) monitoring and follow-up; and (5) on-going evaluation.

**Hours of Work**:37.5 hours weekly - Monday-Friday 8:30 a.m. to 4:30 p.m.

**Requirements**:

- Completed post-secondary education in social services or related field and a current Member of a regulatory college, such as the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Considerable experience addressing the needs of individuals and families experiencing homelessness or those at risk of homelessness including intake and assessments.
- Familiar with Ontario Works and Ontario Disability legislation, Landlords and Tenants Acts etc.
- Understanding of community agencies and a cross-section of service providers.
