Covenant House helps youth ignite their potential and reclaim their lives. As Canada’s largest agency serving youth who are homeless, trafficked or at risk, we offer the widest range of services to hundreds of young people every day. More than a place to stay, we provide 24/7 crisis shelter and transitional housing on-site and in the community, along with comprehensive services including education, counselling, health care, employment assistance, job training and after-care. Since 1982, Covenant House has supported more than 100,000 young people.

**Program Summary**

The Personal Support Worker is part of a new and exciting pilot project at Covenant House Toronto that will prototype occupational therapy and personal support work for young people experiencing homelessness. We will pilot occupational therapy and personal support work focused on supporting young people exiting homelessness to have sustained exits. The pilot project will identify promising practices to support young people with cognitive impairments with the objective of improving housing outcomes and sustained housing. In our pilot project staff will work with people experiencing homelessness with the goal to move towards and maintain stable housing. Occupational Therapists and Personal Support Workers both focus on supporting young people to improve their skills in three areas: leisure, productivity and self-care. They support young people to learn important life skills including budgeting, housekeeping, pet care, and knowledge of landlord-tenant rights and responsibilities. Further, these staff help clients establish and maintain links with necessary services including health care, mental health care, identification, employment, education, and volunteer opportunities. Our pilot will hire one Occupational Therapist and one Personal Support Worker. They will begin to work with young people just prior to existing our emergency shelter or one of our transitional housing programs. They will follow these young people when they move into the community. The objective is to support young people to have sustained exits from homelessness, by offering support in areas where young people with cognitive impairments struggle.

**Position Overview**

The Personal Support Worker (PSW) will support clients to be as safe and independent in their homes as possible, helping them to live their best lives. PSWs provide personal care to clients in their home and may assist them to mobilize and perform light exercises.

**Responsibilities**:

- Review referral documentation and previous related assessments
- During the initial meeting work with young people to identify goals and needs that will be worked on
- Using the assessment to identify the required intervention to meet the needs of young people working with. information to
- Observe young people and their environments.
- Report and document unsafe conditions and behavioral, physical, and / or cognitive changes to supervisor
- Support the reduction of barriers and increase engagement for youth in wellness activities.
- Builds professional relationships with youth to promote trust and safety which allows for growth opportunities
- Communicate and demonstrate basic information in accordance with pre-established Plan of Care (includes tailored interventions to meet the individual needs of young people in their daily living by supporting light housekeeping, meal planning, and preparation)
- Promote youth leadership around development and facilitation of workshops.
- Coordinating and collaborating with the young person’s case management team to reduce barriers and to encourage the development and implementation of leisure, productivity and self-care building opportunities for youth.
- Consulting with others and recommending support for youth through case conferences.
- Evaluate effectiveness of intervention through ongoing assessment using ETO data as well as OSCAR data.
- Build and maintain community partnerships that help to foster community building for
