Working within the context of a culturally safe, client-centered focus the Community LPN is responsible for providing a comprehensive range of nursing services utilizing a population health lens in health promotion and injury prevention. The CLPN will provide care and deliver public health programs and services using a holistic model of care and practice in a culturally respectful and safe manner.
**Scope of Responsibilities**:

- Provides care and service that meets the standards for Registered Nurses within the BC College of Nursing Professionals (BCCNP). Works within scope of practice for Registered Nurses as determined by relevant legislation (Health Protections Act), BCCNP, organizational policy and own clinical competency. Seeks guidance from BCCNP and/or FNHA Regional Practice

Support when required.
- Provides care and acts as an advocate for services that promote and protect the health and wellbeing of individuals, family groups and communities. Invites individuals, families, groups, and communities to be involved as active partners to foster a self-management care approach for chronic conditions.
- Uses evidence and research to inform nursing practice. Collects, assesses, analyses and applies information from various data sources to make evidence informed decisions for nursing services with individuals and families and Seeks out and participates in relevant continuing professional education opportunities to maintain current and competent practice.

**Knowledge, Skills and Abilities**:

- Knowledge of health status of populations, inequities in health, the determinants of health and illness, principals of primary care, strategies for health promotion, disease and injury prevention, health protection, curative, urgent and emergent care, rehabilitation and supportive or palliative care.
- Physical ability to perform the duties of the position.

**Education and Experience**:

- Current registration and good standing with the College of Licensed Practical Nurses of British

Columbia.
- Current Basic Life Support, Level “C” certificate, Complete Criminal Record check.
- 1-2 years recent experience in public health / community health nursing.
- Valid Class 5 Drivers License and own vehicle.
- Completion of immunization certification.
