**Humber River Health. Lighting New Ways In Healthcare.**

Since opening our doors in 2015 as North America’s first fully digital hospital and we remain unwavering in our belief that we can change the hospital where we work, the community where we live, and the world of healthcare beyond our borders. Serving a community of 850,000 residents in North West Toronto, Equity Inclusivity and active participation in the North West Toronto Ontario Health Team are key initiatives important to our Team. At Humber River Health, we use a custom combination of technology and clinical expertise to rebuild elements of care. We make technology work for staff and physicians; giving them more time to spend with patients, to eliminate inefficiencies, and to reduce the chance of errors. Humber River Health is formally affiliated with both the University of Toronto and Queen’s University and committed to becoming a community academic hospital. Clinical Excellence, Optimizing Care through Technology and Community Connection frame our Research Strategy.

At Humber River Health, we’re not hoping for a renaissance, we are making it happen. As part of our dynamic team, you can lead the way, as we continue our journey towards high reliability care

**Join our Environmental Services team as we work together to create a cleaner, safe work environment and patient care setting for our facility**.

At Humber River Health we continue to advance the important role we play as we advocate for healthcare initiatives in the diverse community we serve.

In December 2017, the Central Local Health Integration Network (LHIN), in partnership with Humber River Health, North York General Hospital, Southlake Regional Health Centre, Mackenzie Health and Markham Stouffville Hospital, launched an integrated, innovative model of care delivery called the Reactivation Care Centre (RCC) - A Central LHIN Hospitals Collaborative. The RCC is located in the former Humber River Health Finch site, which was extensively renovated for this new shared facility. Each of the hospitals has its own unit which is staffed and operated as an extension of its hospital. Humber River Health operates one 32 bed unit.

Building on the success of the first collaborative, planning is currently underway for the opening of the second **Reactivation Care Centre** in the** Church site** of Humber River Health that will include a 34-bed patient care unit. The unit will provide specialized care for patients who no longer require acute care, but are waiting in hospital for transfer to an appropriate alternate discharge destination such as convalescent, slow stream rehabilitation, or long-term care.
