**about the role**:
The Hepatitis C Outreach Worker will work as a member of a multi-disciplinary Hepatitis C health team to provide low threshold outreach to members of our community living with/at-risk of acquiring Hepatitis C, connect them to care and support them in achieving health goals.

**day to day responsibilities**:

- Conduct outreach to clients both onsite and in the community to connect clients to the Hepatitis C program for testing and treatment.
- Provide support services to clients in accessing care for Hepatitis C and other social or community service.
- Supports weekly group preparations as required, including food preparation and distribution.
- Provide support and guidance to peer ambassadors at outreach sites
- Work with the Hepatitis C team to coordinate Hepatitis C-related support groups for clients.
- Support Hepatitis C clients along all phases of the Hep C care continuum (including general support, wellness checks, light case management, client advocacy and navigation, and 1:1 support).
- Document all client interactions in the EMR.
- Work with the multi-disciplinary team to support harm reduction programming at Sherbourne, including STASH drop-in.
- Coordinate, design, and deliver education and training sessions.

**To thrive in this role, you’ll need**

**education/experience**:

- Degree or diploma in health or social services or relevant discipline.
- At least 2 years of experience working in a community setting is required or an equivalent blend.
- Thorough knowledge and experience of Hepatitis C, harm reduction, and peer support models.
- Strong knowledge of issues affecting marginalized communities.
- Experience working with diverse agencies and working successfully in partnership with these agencies.
- Current Food Handler’s Certificate or willingness to train.
- Thorough knowledge and understanding of the local health and social service sector, government programs and current legislation that may affect clients.
- Lived experience with population an asset.

- Excellent interpersonal, presentation, and communication skills.
