PSW Job Responsibilities:

- To adhere to the plan of care as determined by the Director of Wellness
- To facilitate residents' move in and transfers where applicable
- To observe and report on residents care/service utilizing appropriate Levante forms and Yardi Systems
- To assist the resident with personal care and hygiene and activities of daily living (morning, evening, bathing)
- To observe and report on changes in residents’ condition to Charge Nurse
- To escalate medical emergencies to Charge Nurse
- To assist with ambulation, mobilization, and transferring
- To demonstrate understanding of residents needs and maximize abilities
- To provide cognitive, social, emotional, and psychological support to the residents/family
- To maintain resident information confidentiality
- To respond to residents/family concerns and ensure appropriate action taken within
decision making authority and or concern communicated to Director of Wellness
- To assist in maintaining a safe and clean environment, including residents equipment, belongings, and nursing equipment
- To observe & report to the Director of Wellness any concerns regarding residents’ safety
- To engage with other agencies involved in resident care as directed
- To work in a position of trust with vulnerable adults (residents)
- To uphold and promote the organization's values and philosophy relating particularly to ethics, morality, and integrity as set out in Levante’s Code of Conduct
- To complete other duties as assigned.

**Job Type**: Part-time
Part-time hours: 10-20 per week

**Salary**: $15.99-$19.20 per hour

Schedule:

- Day shift
- Evening shift
- Night shift
- Weekend availability

Supplemental pay types:

- Overtime pay

COVID-19 considerations:
A minimum of 2 COVID vaccinations required.
