About the Agency: Forward House provides support to adults with an Intellectual Disability to live in their own

Developmental Disabilities Act

Demonstrate independence in decision making

Working with and managing direct support professionals and ensuring all aspects of support meet the Mission,

Principles of Service, Policies and Procedures, MCCSS directives/Compliance/QAM

Ability to communicate clearly, with all team members and community partners

Some experience in using digital forms of communication, documentation

Some experience/knowledge in the Forensic field

Flexibility in hours of work

Participation in on call duties on a regular rotational basis

Qualifications: D.S.W. or equivalent or a B.A. with knowledge of the sector. Minimum of one year of supervisory

experience

Some experience working with individuals who have intellectual disabilities, dual diagnosis and physical disabilities.

The individual should have computer literacy and flexibility when it comes to locations, along with experience in a

unionized environment.

Valid G Driver's License and vehicle is required for this position
