**Job Title: Patient Care Manager**
Location: Bedford, Nova Scotia
At Echelon Wellness, we are dedicated to providing exceptional, veteran-focused healthcare services. Our multidisciplinary clinic is committed to supporting veterans, RCMP and their families throughout their healthcare journeys. We specialize in delivering compassionate care with a comprehensive understanding of Veterans Affairs Canada (VAC) benefits and Medavie Blue Cross coverages.
**Key Responsibilities**:
- Act as the primary point of contact for veteran patients, ensuring they understand and access available healthcare services and coverages under Veterans Affairs Canada (VAC) and Medavie Blue Cross.
- Care Coordination: Oversee patient care plans by coordinating with multidisciplinary teams, including Nurse Practitioners, physical and mental health professionals, and administrative staff.
- Operational Management: Streamline patient intake, scheduling, follow-up, and care documentation to ensure a smooth patient experience.
- Education & Guidance: Educate veterans on their entitlements, benefits, and care options. Act as a resource for families navigating VAC and insurance processes.
- Collaboration: Work collaboratively with case managers, VAC representatives, and Medavie Blue Cross to support veterans in achieving optimal health outcomes.
- Reporting directly to the Director of Operations, Clinic Manager, and C-Suite executives
**Qualifications**:
- Education: Bachelor’s degree in Healthcare Management, Nursing, Social Work, or a related field. Equivalent experience may be considered.
- Minimum 5-10 years of experience in healthcare coordination, patient advocacy, or case management.
- Direct experience with Veterans Affairs Canada (VAC) programs and Medavie Blue Cross is an asset.
- Experience working in a multidisciplinary healthcare setting is a strong asset.
- Exceptional interpersonal and communication skills, with the ability to work effectively with multidisciplinary teams.
- Excellent organizational skills, problem-solving abilities, and attention to detail
- Proficiency in electronic medical records (EMR) systems, Practice Perfect, and MS Office.
