**Role**:

In this role, you will assist a large number of individuals with disabilities and their families and guardians to identify, explore and obtain needed services that will enhance their well-being and assist in their capacity for independence and inclusion in the community.

The main responsibilities of this position will include responding to inquiries, problem solving issues, completing relevant assessments, completing the necessary paperwork and following the different processes to assist the individuals and families to connect to appropriate services. You will collaborate with parents, children, individuals with disabilities, and agency staff to monitor and evaluate the delivery of supports and services and achievement of outcomes. You will also provide key information about program intent and information about options that are available, including eligibility for various programs.

Additionally, familiarity with current relevant legislation and programs, such as Family Support for Children with Disabilities Act, Regulation, policies, procedures, standards and Developmental Disability Act, Regulation, Policies (provincial and regional) is considered an asset.

**Qualifications**:
**Minimum Recruitment Standard**:
A bachelor's degree in social work with some related experience.

**Equivalencies considered**:
Related Education: Master of Social Work (no experience required); or related degrees in Social Sciences with a minimum of two years of related experience; or related diploma and 3 years related experience.

Please note: Successful applicants who possess a BSW, MSW or diploma in Social Work from an Alberta College will be required to register and maintain registration with the Alberta College of Social Workers.

Related Experience: case management, planning and/or arranging services and supports for individuals with disabilities, and/or children, and/or other vulnerable individuals. Experience with service plans, community-based resources, and managing a caseload.

A valid Class 5 operator's license and access to a reliable vehicle is essential as travel is a requirement.

**Salary**:
$2,589.62 - $3,347.57 bi-weekly ($67,330 - $87,037 annually)

**Notes**:
Hours of work: Full Time: 36.25 hrs/week; Monday to Friday work week.
