Youth Habilitation Quinte Inc. requires a Transitional Connector 28 -35 hours per week, who will promote independence and quality of life through the coordination of required support services for young people 16 - 24 years of age. The office will be based in Smiths Falls and the position will service Lanark, Leeds and Grenville.
