**This position is for our Extendicare Oakview Place Home, located in Winnipeg, Manitoba**

**What you’ll be doing**:

- ** Provides a complete resident admission process including tours, admission, discharge, and occupancy tracking.**:

- ** Acts as liaison as appropriate, with families. Provide counselling or group sessions for resident and families as identified.**:

- ** Maintains established department plans, policies, and procedures to meet regulatory and Extendicare standards, including quality assurance.**:

- ** Manages the quality care programs including Guardian Angel, Health Tracking, Behaviour Management, Ethics, and Palliative Care.**:

- ** Participates in providing in-service programs for residents and families as needed.**:

- ** Maintains all required records, reports, statistics, etc. for both residents and department in accordance with policies, procedures, and legislative requirements.**:

- ** Acts in an advocacy role as appropriate.**:

- ** Satisfy Health Record as they pertain to the department (MDS experience an asset)**:

- ** Liaises with government officials, Extendicare consultants, and other professionals in relation to resident care needs and department activities.**:
***What you bring**:

- ** Degree in Social Work from an accredited University preferred.**:

- ** Previous experience in long-term care setting.**:

- ** Knowledge of applicable legislation and regulations.**:

- ** Competency in conducting individual counselling and group sessions.**:

- ** Computer literate**:
