The **Welland McMaster Family Health Team** is committed to provide comprehensive primary care, promote healthy living and inspire excellence and continuous learning. The inter-professional team of health care providers support rostered patients of 11 affiliated Family Physicians and also provides programs to the community at large.
**Position Summary - **The Master Social Worker is a member of an interdisciplinary team and assists in the delivery of the primary care functions of the Welland McMaster Family Health Team. The Master Social Worker provides program development and delivery, patient system navigation, case management and follow-up for youth, adults, couples and families in consultation with affiliated physicians and staff of the Welland McMaster Family Health Team.

**Qualifications**
(i) minimum 3 years experience
(ii) graduate degree in social work from a recognized university;
(iii) member in good standing with the Ontario College of Social Workers and Social Services Workers;
(iv) proof of professional liability insurance and;
(vi) proof of full vaccination required
(vii) Experience with DBT, CBT, IFS, EMDR, FLASH or Brainsinspotting are valued.
**Responsibilities**
**Program Development and Delivery**:
i. development and delivery of evidence-informed workshops, webinars and groups provided by the Welland McMaster Family Health Team and community partners in collaboration with affiliated physicians, nurse practitioners and other interdisciplinary health professionals.
Health Promotion:
i. Post-Partum Screening;
ii. knowledge of community resources and referral pathways.
iii. Back-up triaging
iv. Added asset: foster health and wellness by assessing patient activities of daily living and coordinated access to services in areas such as assistance with daily living, crisis intervention, treatment, health promotion and prevention.
**Case Management**:
i. case management and follow-up on evaluation of achievement of patient goals;
ii. facilitate linkages with appropriate services, supports, and community resources;
iii. involve patients in decisions about their own health;
iv. intake overflow as needed.

Pay: $35.65 per hour
