Transitional Lead

CBI Health is looking for a dynamic and compassionate Transitional Lead to support our program that includes working with an interdisciplinary team that uses an integrated and collaborative approach to service delivery. The Transitional Lead is responsible for working closely with hospital partners to coordinate the effective transition of patients from hospital to home with all applicable home care services provided by CBI Health. Through collaborative work with the hospital lead, patients and families, and other community partners, you will create a holistic plan of care to support patients to transition home and sustainably remain at home. Your is vital in working with the circle of care to ensure the progress of patient care plans, monitoring the progress of patients to achieve identified goals, and identify solutions for issues. Additionally, to measure the effectiveness of the services provided, you will be responsible to ensure that documentation, all data metrics, and outcome measures are captured in a timely fashion as per program standards.
**What you will need to be successful;**
- Registered with the College of Nurses of Ontario in good standing (RN or RPN) or Registered with the College of Physiotherapists in good standing (PT) or Occupational Therapist registered with the College of Occupational Therapists in good standing (OT).
- Clear vulnerable sector screening
- Minimum of 2-3 years of nursing/PT/OT experience
- Experience in home and community care or acute care (community experience is considered an asset)
- Excellent written and verbal communication skills
- High degree of professionalism and patient/family focused care
- Be a team player and be highly collaborative
- Demonstrated clinical leadership & ability to lead discussions and collaboration
- Excellent assessment and coordination skills
- Demonstrated ability to work within an interdisciplinary team
- Strong organization skills and able to prioritize/ multi-task in a fast-paced environment
- Access to a reliable and insured vehicle
- A valid driver’s license and ability/willingness to travel within the Peterborough Region
