**RESPONSIBILITIES**:

- Clinical and Professional Practice _
- Responsible for the provision of Social Work services and MAiD services, for developing, operationalizing, and evaluating the operational plan for the Programs.
- Responsible for the identifying, planning, and facilitating Quality Management and Patient Safety opportunities.
- Responsible for accumulating and assessing various client satisfaction measures and ensuring a plan is put in place to continuously improve.
- Responsible for assuring the care provided meets Professional Standards for Social Work.
- Leadership_
- Responsible for supporting and facilitating patient care teams and interdisciplinary teams to assist in providing patient care.
- Responsible for collaborating with interdisciplinary teams and hospital services and representing the hospital at external committee/ forums to achieve coordinated quality of care and services for patients.
- Responsible for leading and participating with interdisciplinary teams and hospital services to plan, implement, and evaluate special or ongoing projects and initiatives.
- Supporting and enhancing the professional development of the Social Work team.
- To provide leadership for staff through coaching, support, change management, strategic planning, and performance against agreed targets**.**
- Operations Management_
- Responsible for developing, implementing, and monitoring systems to facilitate the effectiveness and efficiency of discharge planning, social work, and MAiD services in the hospital, as well as ensuring all policies and procedures are in accordance with legislation.
- Responsible for coordinating social work staff and services in all areas of the in-and out-patient programs as required.
- Responsible for data collection and monitoring of trends in discharge planning, workload measurement, and to participate in related utilization activities.
- Explore and develop appropriate community linkages with external agencies/ institutions to advocate for new/ or changed services to meet patients needs.
- Responsible for preparing and maintaining capital, operating, and staffing budgets. In partnership with Finance team, develop operating budget and be responsible for monthly variance explanation including unit cost and volume.
- Responsible for responding to, reviewing, researching, and following up with complaints, and incidents.
- Human_ Resources Management_
- Responsible for assessing staffing requirements, as well as hiring, training, scheduling, coaching, motivating, disciplining, and managing staff in accordance with Collective Agreement.
- To be responsible for the completion of the Performance Excellence Program.
- Responsible for managing human and financial resources, including planning resource needs for the service, and working with the Director to plan and monitor operational and capital budgets.
- Responsible for recruitment, retention, and monitoring performance of staff, to ensure professional standards are maintained.
- To provide an environment that promotes staff development and learning.
- Responsible for analyzing workload measurement data and ensuring compliance with regulations.
- Project Management_
- To lead the implementation of new departmental initiatives and projects, ensuring that a strong interdepartmental approach is taken.

**REQUIREMENTS**:

- Master of Social Work from an accredited University.
- Healthcare Management training
- 3 - 5 years related experience.
- Interprofessional working experience
- Project management experience
- Strong interpersonal skills
- IT skills

**PREFERRED**:

- Experience with Meditech
- Membership in the Ontario College of Social Workers
- 5+ years in clinical setting in a leadership role
- Budgetary and performance management experience
- Project management training
