Responsibilities:

- Provide case management services to clients, including assessment, planning, and coordination of services
- Conduct interviews and assessments to determine client needs and eligibility for programs
- Develop and implement individualized service plans for clients
- Monitor client progress and make necessary adjustments to service plans
- Collaborate with other professionals and agencies to ensure comprehensive care for clients
- Maintain accurate and up-to-date client records and documentation
- Provide crisis intervention and support as needed
- Advocate for clients' rights and access to resources

- Strong project management skills to effectively manage multiple cases and tasks simultaneously
- Bilingual proficiency in [languages] to effectively communicate with diverse client populations
- Excellent written and verbal communication skills to document case notes, write reports, and communicate with clients and stakeholders
- Strong time management skills to prioritize tasks and meet deadlines
- Exceptional customer service skills to provide support and assistance to clients in a compassionate and professional manner
- Ability to analyze complex information and make informed decisions based on client needs
- Proficiency in Microsoft Office Suite (Word, Excel, Access) or similar software programs for data entry, record keeping, and generating reports
- Familiarity with computerized case management systems for efficient tracking of client information
- Ability to present information effectively in both individual and group settings

Pay: From $22.00 per hour
