Addictions Coordinator

Back Door Mission facilitates Mission United, a collaborative social service and primary health care HUB for those experiencing homelessness. This program seeks to assist those with high acuity and tri morbidity achieve long term stabilization by providing low barrier, wrap around supports with a point in time model. Through partnerships with various community agencies, we provide specialized person-centered services for our community through a singular access point.

**Location**: Onsite at 66 Simcoe St. S., Oshawa.

**Compensation**: Starting at $23/hour with two weeks' vacation per annum.

**Duration**: September 18, 2023 - August 30, 2024.

The responsibilities and minimum qualifications for this position are as follows:
**Responsibilities**:

- Coordination of Mission United’s Addictions Treatment Continuum.
- Completion of regular reports on program activities and outcomes.
- Researching and communicating with treatment centers for potential partnerships, specifically looking to increase opportunities for pre and post treatment.
- Collaboration with current partners, including education around program model.
- Coordination of client transitions through the treatment continuum.
- Working with MU Clinic and Peer Support Specialist, Addictions, to ensure care plan needs are met by program transitions.
- Advocating the inclusion and accessible addictions care for vulnerable populations to treatment centers.
- Ensure all components of the continuum are achieved.
- Other duties as required.

**Qualifications**:

- Post secondary degree in Health or Social Sciences, or related field.
- 3+ years' work experience with vulnerable populations, with a focus on mental health and addictions.
- Working knowledge of Microsoft Office (Word, Excel, Outlook, Teams, etc).
- Excellent communication skills both written and oral.
- Experience with report writing and data analysis.
- Project management or program coordination experience considered an asset.
- Experience working with EMR systems or HIFIS considered an asset.
- Strong organizational, time management, prioritization, analytical, problem-solving, and conflict resolution skills.
- Able to work with mínimal supervision.
