The incumbent will:
- Manage the nutritional needs of residents through nutritional risk screening, assessments, referrals, nutrition care planning and evaluation of outcomes
- Oversee the nutritional component of the facility's enteral feeding program
- Participate in care conferences with residents and their healthcare advocates
- Develop, implement and maintain policy and procedures specific to resident nutritional care
- Maintain accurate and organized clinical records
- Develop and approve menus to ensure compliance with established menu planning standards
- Plan and conduct health teaching to residents, and educate staff on nutrition related topics
- Participate in quality improvement initiatives
- Mentor students during nutritional care placements
- Actively participate on various committees (i.e.: Home Management, Food Service, Wound Care and Professional Advisory)
**This position will be required to work 3 days per week**
The successful applicant will possess:
- A Bachelor of Science or Applied Science with a Major in Food and Human Nutrition
- Current registration in good standing with the College of Dietitians of Ontario
- Minimum 3 years experience in a long term care setting with extensive knowledge of geriatric nutrition
- Excellent communication, organizational and time management skills
- Demonstrated ability to work independently within an interdisciplinary healthcare environment
- Knowledge of MLTC Program and Accreditation Canada Standards
**Job Type**: Part-time
Part-time hours: 21 per week
**Salary**: $42.95-$53.69 per hour
