POSITION: TECH 4 ACTT PSYCHOSOCIAL REHABILITATION SPECIALIST
Outpatient Mental Health

The Huron Perth Assertive Community Treatment Team is specifically funded by the MOHLTC to provide assertive community treatment and outreach to residents of Huron and Perth counties with long-term serious mental health concerns. Psychosocial rehabilitation will be provided to support those clients whose intensity and frequency of support cannot be met by an existing resource.

REPORTS TO: Manager, Outpatient Mental Health

DATE REQUIRED: As soon as possible

SALARY: $31.31 to $36.05 per hour

QUALIFICATIONS:
**Education**:

- Bachelors degree in Social Sciences or Diploma in Social Services Worker or equivalent preferred.
- Certification in Psychosocial Rehabilitation
- Current CPR certification.

**Experience**:

- A minimum of one year's experience working on an Assertive Community Treatment Team in a rural community preferred.
- A minimum of three year's experience working independently in crisis situations working with the seriously mentally ill client.
- Community experience in supporting the seriously mentally ill client to obtain employment, housing and with finances.

Abilities:

- Demonstrated experience to develop and implement Psychosocial Rehabilitation group activities for the seriously mentally ill client.
- Demonstrated experience to intervene effectively in crisis situations to support the seriously mental ill client.
- Demonstrated success in working to support the seriously mentally ill who is at risk of becoming or who is currently homeless.
- Mature, flexible and self-directed with well-developed time management and organization skills.
- Able to function cooperatively within Huron and Perth multidisciplinary teams.
- Well-developed verbal and written communication skills.
- Experience working independently with seriously mentally ill client, in the home and in the community in the development and support with activities of daily living.
- Demonstrated ability to organize social and leisure group activities for the seriously mentally ill client in community.
- Demonstrated knowledge of the social stressors experienced by the seriously mental ill client in the community.
- Work in collaboration with other mental health team members in providing Psychosocial Rehabilitation.
- Emphasizes relationship building and individualization of services and client choice
- Must hold a valid Ontario driver's license and have access to own vehicle; must obtain and maintain auto endorsement insurance for the purpose of client transportation.
- Computer skills and experience in workload measurement and statistical information.
