The Interprofessional Practice Department has an opportunity for an

**ADVANCE PRACTICE LEADER - LONG TERM CARE**

70 Hours Bi-Weekly| Non-Union | Full Time Days

(some weekends may be required)

**Responsibilities include but are not limited to**:

- Provides expert knowledge to advance care and meet the needs of residents and families, specifically related to responsive behaviours within a geriatric population.
- A focus on improving the knowledge and building competence of clinicians in the Apotex in behavioural care.
- Goals of training all staff in Gentle Persuasive Approach and all registered staff in P.I.E.C.E.S.
- Act as an expert and resource for the interdisciplinary team and provide specialist support to the resident, their family or support network.
- Stay current with Ontario Health, Ministry of Long-Term Care and Home and Community Care relevant policies, directives, legislation and compliance. Collaborates on strategies to address emerging opportunities.
- Work closely with the internal and external Behavioural Supports Ontario (BSO) partners.
- Collaborates with program leadership to ensure the implementation and sustainability of exemplary clinical practices, specifically in responsive behaviour management.
- Collaborates with the RN Clinical Educator on responsive behaviour management education and training.
- Assumes a leadership role within the program, active committee membership and collaborates with interprofessional team to provide exceptional resident/family care.
- Collaborates and contributes to the improvement of resident safety and risk management issues.
- Collaborates with the interprofessional team in the implementation of Required Organizational Practices (‘ROP’) mandated by Accreditation Canada.
- Works collaboratively with other programs and services to ensure quality resident outcomes and effective transitions of resident between programs and care settings.
- Build capacity within point of care staff on emerging best practice and non-pharmacological approach in management of responsive behaviours.
- Contributes to the improvement of clinical outcomes based on best available evidence and efficient utilization of resources.
- Ability to interpret and analyze Rai-MDS data.

**Qualifications include but are not limited to**:

- Five (5) years recent geriatric related experience and expert practice skills
- Gentle Persuasive Approach Coach preferred
- P.I.E.C.E.S. certified
- Master’s degree (post entry-level) in a Health Discipline is required or relevant clinical and academic experience
- Demonstrated experience in leadership and clinical field required
- Current Certificate of competence in good standing from the relevant Regulated Health Profession College (i.e., College of Occupational Therapists of Ontario (COTO), College of Physiotherapists of Ontario (CPO) College of Nurses of Ontario) required
- Membership in the discipline-specific national and/or provincial association is an asset
- Experience in adult education/curriculum development and delivery
- Ability to communicate effectively to direct and motivate team and others, maintaining a persuasive and transparent presentation style at all levels of the organization
- Effective stakeholder engagement and interpersonal skills at all levels of the organization including staff, residents and their families/significant others and other external stakeholders
- Ability to distill complex information into clear, concise messages
- Demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams
- Knowledge of MS Office (Word, Excel and Outlook), Point Click Care (PCC)
- Excellent organizational, planning and time management skills
- Ability to manage multiple, often competing priorities
- Demonstrated effective critical thinking, analytical, decision making and problem-resolution skills
