The Rehabilitation Aide functions as a member of the Home Care Home Independence Team (HIP) and reports directly to the HIP
Lead Therapist.
The Rehabilitation Aide works under the Northern Health Region (NHR) Vision, Mission, Values and Strategic Directions as well as
the provincial Long Term Care (LTC) Aging in Place Strategy.
The Rehabilitation Aide works under the supervision of the HIP Lead Therapist to provide a Restorative Care Treatment Plan for
Home Care clients meeting the program admission requirements. The ultimate goal of this program is to promote client
independence and enabling clients to remain longer in their own homes.
The Rehabilitation Aide has awareness of the chronic disease prevention pillars of the NHR: improved nutrition, increased physical
activity, tobacco reduction and mental well-being.
Finally, the Rehabilitation Aide provides role modeling and education to other Home Care Home Support Workers (HSW) and Health
Care Aides (HCA) in the Restorative Care Approach within the Home Care department.
The incumbent must fulfill the requirements of the Criminal Records/ Vulnerable Person, Child Abuse Registry check and Adult
Abuse Registry check, and adhere to all Northern Health Region policies and procedures.

**QUALIFICATIONS**
- Certification as a Rehabilitation Aide. Combination of education and experience will be considered.
- Two years (recent) directly related experience in Home Care, Long Term Care and/ or Rehabilitation
- Knowledge of Home Care, Long Term Care, Geriatrics and Palliative Care
- Knowledge of the care needs of the elderly, chronically ill, and terminally ill client
- Knowledge and familiarity with Northern health issues, culture, and population to be served
- Knowledge of cultural diversity in Northern Manitoba
- Knowledge of Adult Learning principles
- Safe Patient Handling training
- Valid Class V driver’s license and willingness to travel within the region
- throughout the year
- Comfortable working with computers and Microsoft Office software
- Excellent communication skills
- Demonstrated presentation and teaching skills
- Demonstrates interpersonal skills through clear communication and positive behavior within the Respectful Workplace
