OHMRS Assessor
**Temporary Full Time (Up to xx months)**

**Wage Rate: $30.89 - $ 32.02 Reference#:22-758 A**

Reporting to the Director of Information Management, the OMHRS Assessor is responsible for coordinating and

supporting the team in valid and efficient data entry, and related data collection for patient assessment under the

Ontario Mental Health Reporting System (OMHRS). This role will ensure accuracy of data and distribution of data

within the mandated timelines. The OMHRS Assessor has primary responsibility for the assessment of Mental

Health clients including the coordination of assessment timelines and collection of assessment information,

coordinate assessment completion and complete appropriate section(s) of the OMHRS. Working collaboratively

with the Clinical Decision Support Analyst and the clinical teams, the OMHRS Assessor will ensure the inter
- professional team has access to, and analysis of, any information available through the collection of client

assessments. Additionally, based on this analysis, the OMHRS Assessor will work with the Program Quality Teams

to provide recommendations for ongoing quality improvement of outcomes as well as support the evaluation and

decision-making to maximize the value of care and services provided. The OMHRS Assessor will understand the

key drivers of funding as related to the OMHRS data and identify areas where further information/education is

required.

**Role Responsibilities**
- Assesses and completes appropriate sections of OMHRS assessments as required by Ministry of Health

and Long Term Care and the Canadian Institute of Health Information.
- Works with key personnel (primarily nursing, allied and psychiatrists) confirming they are aware of their

designated tasks and have the needed support and resources
- Ensures that personnel with responsibility to complete a section of the OMHRS have completed this

work in a timely way to support meeting the submission deadlines.
- Provides support to inter-professional teams to review OMHRS data, facilitate effective use of the data

to support patient centred and recovery focused client care and client /family collaboration in
treatment and discharge planning
- Assists the Senior Directors, Program Managers and staff to plan, coordinate and evaluate quality

improvement initiatives as they relate to OMHRS data
- Develops and monitors assessment schedule and notifies manager of any barriers to timely completion

of OMHRS
- Consults and collaborates with clinical teams, Decision Support and site experts to encourage and

facilitate the clinical utilization of OMHRS data

**Required Qualifications and Experience**:

- Diploma in health related profession or field
- Unrestricted registration with an Ontario healthcare regulatory body (ex. College Of Nurses Ontario,

Ontario College of social Work and Social Services Workers)
- Minimum of 3 years’ experience in a health care setting using electronic hospital patient care systems,

health care reporting systems required etc.
- Minimum 1 year recent experience in working with OMHRS data or similar clinical data set from another

sector required
